Chronic fatigue is never one problem. It is always a stack of overlapping dysfunctions.

Underneath chronic fatigue we typically find: Gut dysbiosis or pathogen overgrowth Mitochondrial dysfunction on the OAT Low ferritin or B12 Suboptimal thyroid (often Hashimoto's) HPA axis dysregulation Sometimes mould exposure or chronic viral reactivation

Treating one of these in isolation never works. You can give someone thyroid hormone, iron, and adrenal support, and if the gut and mitochondria are still broken, none of it holds.

This is why we test comprehensively before treating. Blood panel, stool, OAT. Three tests give us the full picture. Then we treat in the correct sequence using the Healing Hierarchy.

Most people with gut issues end up on increasingly restrictive elimination diets. Gluten out. Then dairy. Then FODMAPs. Then nightshades. Then eggs.

Six months later, they can only eat about 15 foods. And they still don't feel well.

Elimination diets work in the short term because they reduce immune load. But they don't fix what made you reactive in the first place. The underlying gut barrier dysfunction, the overgrowth, the inflammation. Those are still there.

Real recovery is about widening the diet back out, not narrowing it further. That only happens when you fix the gut, not just avoid the foods.

Chronic fatigue, brain fog, joint pain, sensitivity to everything. Specialists keep saying nothing is wrong. Have you been tested for mould?

Mould exposure produces mycotoxins. These bind to mitochondria, disrupt the immune system, and drive systemic inflammation. They can take years to leave the body once exposure stops.

38 year old woman. Hashimoto's diagnosed 6 years ago. On thyroxine. Still exhausted, still gaining weight, still flaring.

Her endocrinologist said her thyroid was "well managed." TSH was 1.8. Free T4 in range. But TPO antibodies sitting at 580 (normal below 35). The immune attack was still raging.

We ran a stool test: Calprotectin 145 (normal below 50) Zonulin 138 (normal below 107) Secretory IgA 220 (low) Klebsiella overgrowth

Her gut was driving the autoimmune attack, not the thyroid itself.

Protocol followed the Healing Hierarchy. Crisis stabilisation (sleep, blood sugar, nervous system) for 4 weeks. Then gut clearance with herbal antimicrobials for 8 weeks. Then gut repair. Thyroid protocol came last.

9 months later: TPO antibodies 95. Calprotectin 28. Zonulin 71. Energy back. Weight stable. Flares stopped.

The thyroid wasn't the problem. The gut was.

Healing has an order. Skip the order and nothing holds.

In The Healing Hierarchy I lay out the four levels every chronic illness recovery has to move through:

Level 1: Crisis Stabilisation. Sleep, nervous system, blood sugar. The body has to be safe enough to heal.

Level 2: Gut Restoration. The gut drives immunity, hormones, neurotransmitters, inflammation. Nothing upstream works if the gut is broken.

Level 3: Biochemistry and Detox. Methylation, mitochondria, liver, micronutrients. Restoring the systems that run the show.

Level 4: Hormones and Optimisation. Thyroid, s*x hormones, cortisol curve. This is where most protocols start. It should be where they finish.

The reason your protocol keeps stalling isn't because you're not trying. It's because you've started at the wrong level.

Sugar cravings that won't quit. Brain fog after meals. Recurring thrush or skin rashes. Itching in the ears or scalp.

These can all be signs of candida overgrowth.

Candida is a yeast that normally lives in small amounts in the gut. After antibiotics, sustained high sugar intake, or chronic stress, it can overgrow and start producing metabolic byproducts that drive brain fog, joint pain, and immune dysregulation.

We test for candida overgrowth on both stool testing and the organic acids test. Arabinose on the OAT is one of the clearest markers of yeast metabolism.

Anti-fungal protocols work, but they have to follow the right sequence. Kill candida too aggressively and the die-off response can be brutal.

You've been told your thyroid is "borderline." Or "subclinical." Or "we'll watch it."

If your TPO antibodies are elevated, your thyroid isn't borderline. You have Hashimoto's. An autoimmune attack on the thyroid that drives the dysfunction.

Hashimoto's is the most common cause of hypothyroidism in Australia.

But most GPs never test TPO or thyroglobulin antibodies. They just track TSH and wait until it gets bad enough for thyroxine.

By the time TSH is elevated, the immune attack has been happening for years. Test antibodies early. The earlier you intervene, the more thyroid tissue you preserve.

These can all point to one thing. H. pylori.

H. pylori is a bacterial infection in the stomach lining. It suppresses stomach acid production, which is why iron, B12 and protein absorption all decline. Low stomach acid also drives reflux, because the lower oesophageal sphincter loses tone.

Most GPs only test H. pylori with a breath test when ulcer symptoms are present. We test it on every comprehensive stool test, because it sits silently for years.

Treatment is straightforward. But you have to know it is there.

Bloated after every meal? Reacting to foods you used to tolerate? You might not have a food problem. You might have SIBO.

SIBO stands for Small Intestinal Bacterial Overgrowth. Bacteria that belong in the large intestine have migrated up into the small intestine and are fermenting your food before you can absorb it.

This is why FODMAPs trigger bloating. Why probiotics make things worse. Why gluten-free, dairy-free, sugar-free diets only partially help. You're feeding the overgrowth.

SIBO is diagnosed with a lactulose breath test. It is treatable, but the gut needs to be cleared before food reintroduction can stick. This is why we test, not guess.
