A beautiful before-and-after following both a nasal and temporal pterygium excision.

I performed a wide Tenon’s dissection with conjunctival autografting, achieving an exceptionally smooth postoperative ocular surface and a beautifully restored appearance. Even more importantly, the patient’s vision improved by one line.

As corneal and refractive surgeons, we are incredibly fortunate to practise in an era where advanced corneal-sparing treatments can offer patients alternatives to a corneal transplant.

This is a particularly fascinating case of rare superior keratoconus, where the patient was counselled regarding several treatment options, including DALK, CAIRS, ICL and topography-guided Trans-PRK.

After careful consideration, she ultimately elected to proceed with topography-guided Trans-PRK, with the aim of regularising the highly irregular corneal surface.

Her preoperative best-corrected visual acuity was 6/24. Just three months after a single treatment, her vision has improved to 6/6 corrected, with a dramatically more regular corneal topography. The next step will be to address the small residual refractive error.

Needless to say, the patient — and I — are absolutely delighted with the result.

Cases like this are a wonderful reminder of how far corneal and refractive surgery has come. Being able to restore functional vision and potentially avoid a corneal transplant is incredibly rewarding.

It never gets old. And what an honour it is to be able to offer patients life-changing surgery. 👁️

Ever been told you’re not suitable for laser eye surgery?

Meet one of my recent patients who was -12.00 dioptres and had been told for years that laser vision correction wasn’t an option.

While she wasn’t a candidate for laser, she was an excellent candidate for an Implantable Collamer Lens (ICL).

ICL surgery is an incredible alternative for patients with moderate to very high levels of short-sightedness, correcting prescriptions that are often well beyond the safe limits of laser procedures. The lens is implanted through a tiny incision, the procedure takes around 5 minutes per eye, is virtually painless, and vision typically improves almost immediately.

Seeing the reaction when the patch comes off and patients realise what they’ve been missing never gets old. It’s one of the most rewarding moments in refractive surgery.

I am happy to announce that Crystal Eye and Laser Centre is the first refractive surgery centre outside of the United States accredited to host a Refractive Surgery Fellowship under the auspices of the World College of Refractive Surgery and Visual Sciences.

We are now seeking applications for our Refractive Surgery Fellowship commencing August 2026.
This is a prestigious opportunity for an ambitious ophthalmologist to receive advanced hands on training in refractive surgery.

Upon successful completion of the program, the Fellow will be awarded full Fellowship of the World College of Refractive Surgery and Visual Sciences.

Essential Criteria:
• Fellow of the Royal Australian and New Zealand College of Ophthalmologists (RANZCO)
• Completion of 5th year of ophthalmology training
• Fully registered with AHPRA as a specialist ophthalmologist
Desirable:
• Previous corneal or anterior segment fellowship

You will train in a high-volume, state-of-the-art refractive surgery centre alongside experienced surgeons, mastering the latest techniques and technologies.

Australia remains the world champions in pterygia.

Pre and post op outcomes of a neglected pterygium that advanced over the pupillary margin causing a significant drop in corrected vision of 6/15. After an excision with a wide Tenon’s dissection and sutureless conjunctival autograft, not only was the corrected vision restored back to 6/6 but a stunning post op cosmetic result was also achieved.

Perth and Western Australia’s First Planned Combined CAIRS + ICL Surgery – Life-Changing Result! 👏.

Proud to share that we successfully treated a patient with advanced keratoconus who went from 20/400 unaided vision (best corrected 20/100) to 20/20 UNAIDED vision.
Prior to surgery the cornea was extremely steep and irregular. We first performed CAIRS (Corneal Allogenic Intrastromal Ring Segments), which beautifully regularised the cornea. This allowed us to achieve 20/20 corrected vision, but a large refractive error remained (as anticipated). We then implanted a customised ICL to correct it.
The patient is now seeing 20/20 without glasses — going from legally blind to normal vision.

A powerful example of how modern, staged corneal and lens technologies can deliver exceptional outcomes in complex cases.

A patient presented with advanced keratoconus in his right eye—unaided vision of 20/400, and even with glasses, only reaching 20/150. Daily life was a challenge, with vision that most would find profoundly limiting.

Following CAIRS corneal transplant surgery, the transformation has been nothing short of remarkable.

Today, he sees 20/40 unaided—and with correction, achieves 20/20.

Even more striking than the numbers is the structural change: a cornea that was once severely ectatic is now regular and stable. What was once a progressive, vision-distorting condition has been reshaped into clarity.
