Part 4 of a remarkable transformation: Stabilising the Mandible.

The ultimate goal of the ARCHMORE process is an uncompromised final restoration, which serves as the driving force behind every stage of digital implant planning. In this fourth chapter with Oral and Maxillofacial Surgeon Dr. Ricky Kumar and Prosthodontist Dr. Bevan Chong, the focus shifts to the complex anatomy of the mandible.

Because managing an edentulous mandible presents unique stabilisation challenges even with soft tissue-borne support our team designed a highly strategic two-guide approach to eliminate lateral movement entirely:

Guide 1 (Initial Alignment): Utilises a precise tissue punch mechanism allowing for the immediate placement of implants at positions 35 and 45 to secure the foundational architecture.

Guide 2 (Rigid Fixation): Following an anterior flap elevation from 35 to 45, this second guide locks directly onto the surgical site with fixation pins. The result is total structural stability with zero lateral movement, providing a solid platform for the remaining anterior implants.

Nerve Proximity Management: To safeguard the mandibular nerve, 8mm implants were chosen for the posterior region. While the anterior region typically allows for longer fixtures, the anatomy here dictated shorter, wider implants placed with absolute intent to respect anatomical boundaries while optimising retention.

With both arches structurally secure and biologically respected, the foundation is officially set for a life-changing final result.

Part 3 of a remarkable transformation: Surgical Ex*****on & Stability.

The ex*****on of a flawless plan. In this next chapter of our ARCHMORE case with Oral and Maxillofacial Surgeon Dr. Ricky Kumar and Prosthodontist Dr. Bevan Chong, we move from structural guidance to implant placement and bone contouring.

The precision design of the M GUIDE system streamlines the active surgical phase:
* Why All-on-6? Thanks to an abundance of bone volume, an All-on-6 approach was selected over All-on-4. Placing six implants offers maximised stability and optimal retention for the final prosthesis.
* Angled Multi-Units: To accommodate the natural architecture, implants at positions 14 and 24 were placed with angled multi-unit abutments.
* Ridge Contouring: This intricate case required substantial bone reduction. Once all six implants were successfully integrated into the maxilla, Dr. Kumar levelled off any remaining excess bone across the ridge to create a perfectly uniform foundation.

With the upper jaw completed, the stage is set for seamless prosthetic delivery.

Precision is everything when rebuilding a smile after severe trauma. In this next stage of our ARCHMORE case with Oral and Maxillofacial Surgeon Dr. Ricky Kumar and Prosthodontist Dr. Bevan Chong, we dive into the dual-guide system designed for maximum stability in an edentulous patient.

To ensure the highest accuracy for the implant placement, our team at MoreDent reverse-engineered a two-part surgical guide system:

Guide 1 (Tissue-borne): Placed directly on the tissue to accurately drill and secure three vital fixation pin holes.

Guide 2 (Bone-supported): Inserted after the flap is raised, securely locked into place by the fixation pins for completely stable, uncompromised drilling.

By overlaying the patient’s current denture onto the digital implant planning software, we can ensure the emergence profile perfectly aligns with the final bridge. This meticulous blend of digital precision and collaborative surgical insight guarantees optimal form, function, and long-term results.
