🎉 **Celebrating Our eReferral Champions: Norfolk Family Health Team!** 🎉

This week, the Brantford Brant Norfolk Ontario Health Team (BBNOHT) is proud to recognize **Norfolk Family Health Team (NFHT)** as our **eReferral Champions**!

NFHT has been using **Ontario Health eReferral** for over five years, demonstrating a longstanding commitment to improving access to care and enhancing the referral experience for both providers and patients.

eReferral is a secure electronic referral system that replaces traditional fax-based referrals with a standardized, digital process. It enables:

* Improved access to care
* End-to-end referral tracking and visibility
* More efficient communication between referring providers and specialists
* Reduced administrative burden and fewer lost or delayed referrals

Ontario Health has set a goal of having all primary care providers, community-based specialists, hospitals, and mental health and addictions providers using eReferral by March 31, 2027. Organizations like NFHT are helping lead the way by embracing digital tools that support more connected, efficient, and patient-centred care.

The Mobile Cancer Screening Coach is coming to Brant (Paris) this weekend! Taking care of your health has never been easier. Visit the Mobile Cancer Screening Coach at the Community Connections Day at 70 Laurel St., Paris on Saturday July 25th from 10 am to 4 pm.
Services available include:
* Cervical cancer screening (ages 25 to 69)
* Mammograms (ages 40-74)
* At home colon cancer screening kits (ages 45-74)
* Smoking cessation support and ni****ne replacement therapy (ages 18+)
Walk-ins are welcome on a first-come, first served basis or book ahead by calling 905-975-4467.

BBNOHT and Grand Erie Public Health are inside the supporting the Cancer Screening Mobile Coach that's in the parking lot.
If you need Cancer Screening, the coach is open from 9-3:00 and you can be screened for colorectal, cervical and breast cancer. All you need is a healthcard or your interim federal health benefits! No appointment needed.

When you are done with your appointment, come on in and chat with us. BBNOHT is helping our community get paired with family doctors in our area!

The Mobile Cancer Screening Coach is coming to Brantford tomorrow!
Taking care of your health has never been easier. Visit the Mobile Cancer Screening Coach at the Wayne Gretzky Sports Centre in the parking lot (254 N Park St.) on June 25 from 9 a.m. to 3 p.m.

Services available include:
✅ Cervical cancer screening (ages 25–69)

On Friday, May 1, an important announcement was made by MPP Will Bouma highlighting a major investment in local healthcare. The provincial government is investing more than $2.4 million to help connect over 6,389 people in Brantford Brant and Norfolk to primary health care.

This investment will connect more residents with primary care providers and expand access to team-based care across Brantford, Brant, and Norfolk. The Brantford Brant Norfolk Ontario Health Team continues to work closely with the Grand River Community Health Centre, Norfolk Family Health Team, PrimaCare Community Family Health Team and our Primary Care Network on a coordinated plan to scale up primary care in our communities and attach everyone to primary care by 2029.

What this means for our community:

More people connected to family doctors and care teams
Expanded access to comprehensive, team-based primary care
Continued progress toward ensuring everyone has access to care close to home
