In occupational therapy, we evaluate both environmental and cognitive barriers to daily occupations. For clients with chronic insomnia, bedtime often triggers maladaptive thoughts that activate the sympathetic nervous system, causing physiological hyperarousal.

Cognitive Restructuring is a core component of CBT-I designed to systematically identify and reframe these cognitive distortions.

The Clinical Mechanism:
-Identify: Target automatic, catastrophic thoughts (e.g., “I cannot function tomorrow without 8 hours of sleep”).
-Examine: Evaluate the objective validity of the belief based on past performance data.
-Reframe: Replace the distortion with a balanced, neutral alternative to reduce performance anxiety and lower autonomic arousal.

The Clinical Reframe:
❌ “My health is actively failing because I can’t sleep tonight.”
✅ “My body is capable of resting while awake. I have successfully managed previous days on restricted sleep.”

By modifying the cognitive environment, we remove the internal barriers preventing the nervous system from transitioning into a restful state.

CBT‑I is not another sleep tip or supplement. It’s a structured medical protocol with decades of evidence. 70–80% improve, often within weeks.

💤(Fear of sleep restriction) “I can’t handle less sleep.”
Sleep restriction is controlled, temporary, and designed to increase deep sleep. Clients are guided step‑by‑step.

This hit me in the feels today. I got this message, out of nowhere, from a colleague.

I like to think I’ve always been collaborative, but calm and insightful would not have been words to describe me 20 years ago.

I believe personal development helps professional development, and damn, I am constantly working on that. I also believe that exhibiting positive traits at work allow others to mirror those traits.

Make work fun. Laugh. (Swear.) Make jokes. (Wear a NASA t-shirt with a Fozzie bear sweater). Take your breaks. Love your occupation.

…and thank you to this nurse…It’s a gentle reminder how small actions can brighten a world (or this basement cubicle I’m sitting at this morning).
