It’s not uncommon in my practice to run into clients, especially those with intense careers like first responders and military personnel that have a hard time with emotional access.
These clients often report feeling numb, shutdown, burnout or “fine” - and other times where they experience intense feelings all at once, like a panic attack that comes out of nowhere or an anger outburst. They live in emotional extremes, and tell me they go from “0 to 100.”
So how can we as therapists help these clients? I emphasize establishing safety and rapport building, history taking that not only focuses on their OSI (operational stress injury) but also inclusive of their attachment wounds, thorough assessment of dissociation, and effective “front loading” before moving into phases 3-8. This can look like affect management strategies and tolerance building, parts work and more!
