Wolf Creek PCN Welcome to the Wolf Creek PCN! We are part of your Patient Medical Home! PCNs develop solutions to meet the needs of the local community and their health issues.

Primary Care Networks (PCNs) bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. PCNs are comprised of groups of family physicians working with other health care professionals such as nurse practitioners, nurses, dietitians, pharmacists, social workers, exercise specialist, behavioral health consultants, and mental health professi

onals. PCNs are created through an agreement between physicians and Alberta Health Services. Please note that PCNs are not a governing body to physicians; the College of Physicians & Surgeons of Alberta regulates the practice of medicine in Alberta. There are 40 PCNs operating throughout Alberta with more than 3,800 family physicians, and more than 1,400 other health practitioners. At any given time, there are hundreds of PCN family doctors across Alberta accepting new patients. Research shows that patients who regularly visit a family doctor are healthier and live longer. The Wolf Creek PCN supports the Central Alberta communities of Bentley, Delburne, Innisfail, Lacombe, Ponoka, Rimbey, and Sylvan Lake. Back to school can mean more asthma flare-ups for kids, thanks to increased exposure to viruses, allergens and other triggers. Follow these five tips to avoid what's known as the "September Spike":

✅ book an asthma check-up for your child before school starts
✅ get back into consistent medication routines and ensure your child always has access to their inhaler
✅ help your child understand their asthma triggers and how to avoid them
✅ ensure the school is aware of your child's action plan in the event of an asthma attack
✅ keep your child home from school when they're sick

Watch for early symptoms and seek medical advice as needed from your primary care provider and PCN team

As kids and teens head back to school, they may face new social situations and peer pressure. One of the best ways to help them navigate these moments is to start the conversation early.

Here are a few ways to prepare your child:
🚗 Have a plan. Let them know they can always call or text if they need a safe ride home.
💙 Build confidence. Remind them that it's okay to walk away from situations that don't feel right.
👂 Keep the conversation going. One talk isn't enough. Regular, judgment-free check-ins help build trust.

Hot weather can quickly become dangerous if you don't recognize the signs of heat-related illness. Do you know the difference between heat exhaustion and heat stroke?

Heat exhaustion may cause:
💧 Heavy sweating
😓 Dizziness or weakness
🤢 Nausea
🥵 Cool, clammy skin

Move to a cool place, drink water and rest.

Heat stroke is a medical emergency. Call 911 if someone has:
🚨 A body temperature above 40°C
🚨 Confusion, slurred speech, or loss of consciousness
🚨 Hot, red skin

If you have questions about staying healthy during the summer months or managing health conditions that may increase your risk during extreme heat, talk to your primary care provider.

🌿 Enjoying Alberta's trails, parks and backyards this summer? Make tick checks part of your outdoor routine.

Ticks can be found in grassy and wooded areas and some can carry diseases that may cause illness. After spending time outdoors, take a few minutes to check your skin, clothing and pets for ticks.

Watch for symptoms such as a rash, fever, fatigue or muscle aches and seek medical advice if they develop.
