The Institute for Safe Medication Practices Canada is an independent national not-for-profit organization committed to the advancement of medication safety in all healthcare settings. We work collaboratively with the healthcare community, regulatory agencies and policy makers, provincial, national and international patient safety organizations, the pharmaceutical industry and the public to promote

safe medication practices. ISMP Canada's mandate includes analyzing medication incidents, making recommendations for the prevention of harmful medication incidents, and facilitating quality improvement initiatives. ⚠️ Teva Canada Limited is recalling one lot of the non-prescription drug Novo-Gesic Forte 500 mg (acetaminophen) due to the potential presence of phenolic resin or epoxy which, if ingested, may cause injury to the digestive system. The affected product was supplied to pharmacies in 1,000-count bottles and was dispensed to patients in smaller quantities.

L’ISMP Canada mène présentement une évaluation indépendante afin d’examiner dans quelle mesure le travail de l’ISMP Canada a contribué à améliorer la sécurité des médicaments à travers le Canada. Les conclusions de cette évaluation serviront à favoriser l’apprentissage organisationnel, la responsabilisation, l’amélioration continue et la planification future.

Ce questionnaire porte sur votre connaissance et votre utilisation des ressources et des activités de l’ISMP Canada en matière de sécurité des médicaments, sur la manière dont elles vous ont servi dans votre travail, ainsi que sur leur influence sur les pratiques et les améliorations en la matière. We are undertaking an independent evaluation to examine how ISMP Canada’s work has contributed to improving medication safety across Canada. The findings will support organizational learning, accountability, continuous improvement, and future planning.

This survey asks about your awareness and use of ISMP Canada's medication-safety products and activities, how they have informed your work, and their influence on medication safety practices and improvements. The results will help assess the reach, usefulness and contribution of ISMP Canada's work and identify opportunities for future improvement.

The survey should take approximately 10 to 15 minutes, depending on your responses. You may skip any question that you do not wish to answer.

🚨 Attention Community Pharmacists!

❓Did you know methadone was among the top 10 medications involved in harm-related incidents reported to the NIDR in 2025?
The NIDR has received reports describing 10-fold methadone dosing errors involving prescriptions for small volumes/doses of methadone (e.g., doses where a total volume of less than 1 ml of stock solution is required).

This highlights the importance of adopting the following strategies to mitigate the risk of this type of error:
✅ Conduct independent double checks during the preparation step to ensure correct dose calculations and measurements
✅ Confirm with each patient the medication and dose they are expecting

🚨 Attention: hospital care providers and community pharmacies

A recent report to the NIDR described an incident where a hospital discharge prescription was sent to a community pharmacy; the anticoagulant dose was changed, and the discharge prescription was then re-sent. However, without the change noted, it was assumed to be a duplicate, and discarded. The previous, incorrect dose of the anticoagulant was dispensed, which led to the patient experiencing harm and being readmitted to hospital.

Recommendations to prevent these types of errors:
Hospital care teams:
• Incorporate start, stop, and change sections/designations on discharge prescriptions and/or discharge summaries to assist with medication reconciliation in the next care setting.

Community pharmacy teams:
• Implement an independent double check of dispensed medications against discharge prescriptions.
