AI is showing real promise in supporting chronic disease management in primary care. A recent pilot led by the , in partnership with , the Mid-West Toronto Family Practice Network, OntarioMD, and , tested an AI-enabled clinical decision support tool to improve diabetes care.

The tool, based on the ’s clinical guidelines, helped identify patients at risk of Type 2 diabetes, uncover gaps in care, and support evidence-based decision-making using EMR data. Read more at the link in bio
