Collaborative Practice for Patient Safety
Introduction
Patient safety depends not only on the availability of safe clinical practices and technologies, but also on how healthcare professionals, patients, and other stakeholders work together to make and implement safe decisions. Collaborative Practice for Patient Safety—IPPSEP Framework II illustrates this integrated approach by linking the social system of shared decision-making with the technical system of safe care delivery. The framework emphasizes that these two systems must operate together, and within the appropriate setting, level, and context, to translate collaborative decisions into safe care and ultimately better outcomes. Explanation of the illustration
The illustration presents Collaborative Practice for Patient Safety through IPPSEP Framework II as an interaction between two complementary systems. The social system, based on shared decision-making, brings together working in teams and engaging patients to produce safe decisions. The technical system, represented as a delivery model, combines safe practices, using informatics, and improving safety to achieve safe delivery. Both systems operate within the relevant setting, level, and context, and converge in safe care, which aims to produce a better outcome. The feedback and feedforward loops illustrate how outcomes continuously inform both decision-making and care delivery, supporting ongoing improvement in patient safety. IPPSEP II.pdf
