A little glimpse behind the clinic door.

After a while, you get to know people. What they like, what they don’t, what we might tweak and what we’re absolutely leaving alone. There’s trust, a lot of chat, and usually a bit of laughter in between.

13 years of injecting and seeing someone genuinely delighted with the result still doesn’t get old.

My current evening pigmentation protocol.

Summer wouldn’t usually be my first choice for tackling pigmentation, but my brother gets married soon and I’d like to give my skin a nudge in the right direction.

I don’t dabble. These are prescription-only treatments, prescribed following consultation because they’re potent and not appropriate for everyone.

🚀Tretinoin accelerates epidermal turnover, improves the distribution of existing pigment and stimulates collagen.

🚀Hydroquinone 4% remains the benchmark topical for suppressing melanogenesis by inhibiting tyrosinase.

🚀Azelaic acid 15% tempers inflammation while modulating pigment production, making it particularly useful for post-inflammatory hyperpigmentation and melasma.

Pigmentation is one of the most tenacious conditions we treat. Once melanocytes have been repeatedly provoked—by UV exposure, hormones or inflammation—they become far more readily reactivated than subdued. It’s why recurrence is so common, particularly in melasma.

And finally, the least glamorous but arguably the most important product of all: SPF. Without meticulous photoprotection, even the best prescription regimen is working uphill.

A treatment I’m doing much less of these days is tear trough filler.

Not because it doesn’t work. In the right person, it really does.

But it became a treatment that was offered far too freely, and I’ve seen enough disappointing results to be much more selective.

More often, I’m looking at skin quality first with treatments like polynucleotides.

The biggest green flag? A doctor who’s happy to tell you you’re not the right candidate.

I genuinely share in the excitement when someone sees their goals come together in a natural, balanced way.

A real pleasure to treat this wonderful lady, who travelled all the way from North Wales to see me.

The goal was balanced, harmonious rejuvenation — never treating one area in isolation, but assessing the face as a whole. Thoughtful restoration through the temples, mid face, lips, chin, marionette and jaw/preauricular areas created a fresher, more refined result while still looking entirely like her.

She was delighted with the outcome — and so was I.

Grateful, as always, for the expert guidance of and to for such beautifully versatile products to work with.

I little operation yesterday, nothing serious, and feeling good, but: Here’s plan for scar healing.

A quiet reminder: scars can take up to two years to properly remodel — and most scarring is governed by genetics and skin type, so comparison is rarely helpful.

At the start
Clean. Supported. Low tension.
Glue left entirely to its own devices.
No picking. Ever.

Once the skin is properly healed
I’ll introduce silicone gel. There’s enough solid evidence behind it for early redness and thickness that I’m happy to use it — alongside keeping the area moisturised and daily SPF when uncovered (particularly relevant with a sunny February escape on the horizon… very ready).

At this stage, gentle massage with an emollient can also help with comfort and pliability.
If it didn’t settle as hoped
Hypertrophic or keloid scars are a different pathway altogether — and one where medical treatments such as topical or intralesional steroids may be appropriate.

Much later, if I want to refine
Microneedling can be genuinely helpful for improving texture, blending and overall scar quality once everything is flat, calm and mature.

And finally — scars don’t always need erasing.
I don’t mind them, and I’m never averse to blaming mine on a shark bite if the moment calls for it.

A new joined-up approach to skin and hormone care.

Over time, the same conversations keep coming up.

Women come in to talk about their skin — and end up asking about hormones.
Women come in to talk about perimenopause or menopause — and ask what’s happening to their skin.

It happens often enough that bringing these conversations together simply made sense.

So this felt like a natural next step: joined-up skin health and hormone care for women moving through perimenopause and menopause.

This brings together skin-focused medical care at The Curated Clinic with specialist menopause care from Dr Sabrina Ong.

Dr Sabrina Ong is a BMS-certified menopause specialist, and between us we have over 35 years of medical experience. This collaboration has grown directly out of what women are asking for — care that looks at the whole picture, rather than separating skin and hormones into different conversations.
