How much eyelid should a blepharoplasty actually reveal?

There isn’t one answer.

For this 54-year-old patient, keeping some of her natural upper eyelid fullness was particularly important. She had always had relatively full upper lids and didn’t want surgery to change that characteristic of her eyes.

Before surgery, we looked through older photographs together to understand how her eyes had looked over the years — and, importantly, how much upper eyelid show felt right for her.

We therefore planned a deliberately conservative upper blepharoplasty, removing enough skin to refresh the eyes while preserving the fullness that has always been part of her appearance.

These photographs are just 7 days after surgery, so there is still bruising, swelling and redness around the incisions. The result will continue to settle considerably over the coming weeks and months.

For me, good eyelid surgery isn’t about creating the maximum amount of eyelid show. It’s about finding the right amount for the individual.

Every eyelid operation starts long before the first incision.

That’s exactly what this patient wanted.

Two months after upper eyelid surgery, her eyes look brighter and more refreshed, while still looking completely natural.

One of my favourite comments from her was:

“Now I look like I’ve had a great night’s sleep… every day.”

For me, that’s what successful eyelid surgery should achieve.

Not a different face.

Not “surgery eyes.”

Just a rested version of yourself.

Results like this don’t come from removing as much skin as possible. They come from careful planning, preserving natural eyelid anatomy, and understanding that subtle changes often have the biggest impact.

Sometimes the best results are the ones nobody can quite put their finger on.

This lovely patient in her 70s wanted a very conservative approach — not to look different, but simply to feel and look more rested.

At just 10 days after surgery, there is already a brighter, more open appearance to the eyes, while still looking completely natural. There is still some residual swelling and healing to come, and the final result will continue to refine over the next few months.

My philosophy is always to respect the patient’s anatomy and goals, particularly when patients are seeking subtle rejuvenation rather than dramatic change.

One of the most common questions I get asked before upper eyelid surgery is:

“How long will recovery take?”

These photographs were taken just 7 days after upper eyelid blepharoplasty, immediately after suture removal.

At this stage, some swelling and bruising is completely normal, but most patients are already starting to appreciate a lighter, more open appearance around the eyes.

Recovery is a process, and final results continue to refine over the following weeks and months.

Every patient heals differently, but understanding what to expect can make the decision to undergo surgery much less daunting.

If you’re constantly raising your eyebrows to see more clearly, it may not be ‘just ageing’

This 79-year-old gentleman came to see me because his upper eyelids had gradually become heavier over the years. Like many patients, he assumed it was simply part of ageing.

In reality, he had a combination of ptosis (drooping of the eyelid itself) and excess upper eyelid skin, both of which were restricting his superior field of vision.

The last photo shows his journey from:

The difference isn’t just cosmetic.

Notice how much more open the eyes appear, and how the forehead muscles have relaxed. Before surgery, he was constantly raising his eyebrows to help lift the eyelids and improve his vision. After surgery, that effort is no longer needed.

Patients often tell me that the biggest benefit isn’t how they look—it’s how much easier it becomes to read, drive, watch television and simply go about daily life without feeling they’re constantly looking through a curtain.

Flying visit to .uk annual conference today to speak about the importance of eyelid function in dry eye disease.

Most people think dry eye is just about tears. It’s not.
Your eyelids — and how you blink — are just as important.

Every blink spreads a fresh tear film, protects the surface of the eye, and keeps vision clear and comfortable. When that blink is incomplete or the eyelids don’t sit or move properly, symptoms persist… no matter how many drops you use.

Upper eyelid heaviness isn’t just cosmetic.

This patient came in with:
• Constant forehead tension
• Eye strain by the end of the day
• A feeling of “working to keep the eyes open”

After surgery:
• Lighter, more open eyes
• Reduced need to recruit the forehead
• A more relaxed, natural appearance

Look closely at the forehead in the after —
less frontalis overactivity, smoother resting position.

That’s the functional benefit most people don’t realise they’ll get.

Recovery:
• Bruising at 1 week (see final slide)
• Back to normal activities quickly
• Final result continues to refine over weeks

If your eyes feel heavy by the end of the day, it’s worth assessing — it’s not always “just ageing”.
