Someone cuts you off in traffic, and suddenly you're gripped by a rage that feels way out of proportion.

A flash of irritation makes total sense. But the intensity of that rage often isn't really about the other driver.

As psychotherapist Ahi Wheeler at Harley Therapy explains, it can be an old wound resurfacing - being dismissed, overlooked, or made to feel invisible - finding its target in that moment.

Ever felt swept away by an emotion that seems way bigger than what's actually happening?

That might be an emotional flashback - past feelings resurfacing in the present, often disproportionate to what's in front of you.

As psychotherapist Ahi Wheeler at Harley Therapy puts it: "If it's hysterical, it's historical."

as Dr Naresh Buttan, consultant psychiatrist and ADHD specialist, explains, adult ADHD doesn't always look like the stereotype

it can look like struggling to manage money and sliding into debt without meaning to

it can look like restlessness, not being able to sit still, jumping up mid film to check your phone, wander into the garden, play with the dog, then come back and rewind because you've lost the thread

it can look like reading the same three pages over and over, needing a marker just to find your place again

it can look like starting a task, leaving it unfinished, moving to the next, leaving that unfinished too

or the twenty tabs open on your laptop, all at once, none of them closed
if any of this sounds familiar, it might be worth talking to someone

as Dr Naresh Buttan's patient described it, imagine an orchestra where every instrument knows when to play, what octave, when to stop, all led by one conductor keeping everyone in sync

now imagine that conductor falls asleep

everyone still plays, just all at once, no timing, no order, just noise

that’s what it can feel like inside an ADHD brain, not laziness, not carelessness, just a busy, brilliant mind without a conductor to guide it

if this sounds familiar, you're not alone, and it's not your fault

In ADHD relationships, a pattern often forms: one partner ends up pursuing, nagging, or even parenting the other - not because they want to, but because things keep slipping. But underneath that "nagging," there's usually a quieter truth: "I don't feel like you're holding me in mind. I don't feel like I matter." Full video on our YT channel 🔗

"I feel like I don't matter. I feel lonely." Sound familiar? In couples where one partner has ADHD, both people can end up feeling this way, one feeling like they're constantly disappointing, the other feeling unseen. The real work isn't fixing ADHD, it's healing the cycle of hurt underneath it. Full video on our YT channel.

When one partner is neurodivergent (like ADHD) and the other is neurotypical, communication can get complicated fast. This is often linked to the "double empathy problem," where both people understand they process things differently, but still struggle to navigate it together.

The result is a negative cycle of interactions that pulls partners apart instead of bringing them closer.

In therapy, we work to de-escalate that cycle, recognise the positions each partner falls into, and slowly rebuild a new pattern: one of connection and repair.

It's not always about germs. Sometimes it's doubt. Sometimes it's fear. OCD shows up differently for everyone, and left unchecked, the anxiety it creates can spiral into depression. Full video on our YouTube channel 🔗

A black cab driver spending 4 hours every morning checking every route he'd driven, terrified he'd hit someone without realising it.
This is what harm OCD can look like.
With the right treatment - therapy and medication - he recovered.
If intrusive thoughts about causing harm feel like they're taking over, support is available.
Dr Naresh Buttan is a Consultant General Adult Psychiatrist at Harley Therapy. Case example shared with permission.
