Let’s take a look at a more complicated case. This patient had previously undergone two rhinoplasties elsewhere. Her septum was deviated with asymmetries and a low, bulky nasal tip. Her nose also had a dorsal hump and overall looked heavy against her otherwise pleasing facial features. The diagrams show the underlying nasal structures.

So let’s see this in real life. This patient came for a primary rhinoplasty to address excess tissue in all components of her nose creating a dorsal hump and a bulky, undefined tip. All these components were reduced and her tip suspended to prevent drop. This left excess skin which was supported at the tip by cartilage grafting to give definition. She worked hard with massages post-op.
Result at 6 months, later results to follow.

Reducing these left excess skin and this was supported. y tip grafting as in the diagram. Results shown at 6 months. Results at 12 months+ to follow

Let’s start with a bit of basic anatomy- useful to understand what happens in a rhinoplasty. First imagine the nose with two sidewalls held up by a supporting partition which is called your nasal septum. The sidewalls have 3 parts- bones, middle cartilages and tip cartilages. These structures give the skin and hence your nose its shape. Overgrowth as shown here can give the nose a dorsal hump, heavy or long appearance.
