When rhinoplasty draws attention for the wrong reasons, revision surgery can restore facial harmony and confidence.

This patient underwent rhinoplasty over 20 years ago, but was left with a nose that looked too up-turned and unnatural.

Revision rhinoplasty required rebuilding the nose using rib cartilage to lengthen the nose, reduce projection and recreate balanced proportions.

The greatest challenge wasn’t the cartilage reconstruction - it was the scarred, contracted skin envelope that had spent three decades adapting to the previous framework. Careful tissue handling was essential to safely allow the skin to stretch safely over the new shape.

Revision rhinoplasty is often one of the most demanding procedures in plastic surgery. Success depends not only on technical precision but also on a deep understanding of tissue behaviour gained through reconstructive plastic surgery.

These photographs are just 2 months after surgery, with further refinement expected as swelling continues to settle. It was so great to see what a difference the change had made to her confidence even at this early stage.

The aim is simple: a nose that complements the face rather than dominates it.

Restoring the male jawline.

This gentleman was troubled by progressive loss of jawline and neck definition. His goal wasn’t to look different - but to regain a sharper, more masculine profile while maintaining a completely natural appearance.

He underwent a bespoke deep plane facelift and deep neck lift, combined with:
• Parotid gland reduction to restore the lateral jaw angle.
• Functional correction of long-standing facial asymmetry following previous facial palsy using selective platysma myectomy and neurectomy.

The result is a stronger, more defined jawline, improved facial balance and a natural rejuvenation without changing identity.

One point worth highlighting is the scar. Even in men with little or no hair to camouflage the incision, meticulous surgical planning and closure can leave scars that remain remarkably inconspicuous.

Every face is different. The best facelift isn’t oneoperation performed on everyone
- but a bespoke approach based on a detailed understanding of anatomy to achieve the most natural result and the strongest, most defined jawline possible.

A heavy jawline is rarely caused by just one thing.

Most people think it’s simply “jowls”, but there are three key anatomical structures that commonly contribute to heaviness along the jawline:

• Jowls – descended facial tissues that have fallen with age and gravity.
• The parotid gland – which wraps around the back of the jaw and may become more prominent beneath with age.
• The buccal fat pad – which can descend into the lower face, creating fullness in front of the jowl.

A deep plane facelift allows these deeper structures to be addressed through a single operation. By repositioning the descended tissues and, where appropriate, contouring the deeper anatomy, it is possible to restore a smooth, well-defined jawline.

The best facelift is created by understanding anatomy and applying a bespoke approach to achieve a natural, well-defined jawline.

Not every neck needs a facelift.

Advanced neck contouring is not just for facelift patients.

This 38-year-old patient was happy with her face but increasingly bothered by the fullness beneath her chin and the lack of definition between her jawline and neck.

She had a youthful face, so a facelift wasn’t the right operation.

Instead, we performed an isolated deep neck reduction, addressing the deeper anatomical structures responsible for the fullness to restore a sharper jawline and a more defined neck contour - without changing the character of her face.

The aim was not to make her look different, but to reveal the neck contour that had been hidden beneath the deeper tissues.

Advanced Neck Contour & Deep Plane Facelift Surgery

This lady in her mid-50s came to see me because she was bothered by her heavy neck, full lower face and jowls. Like many patients, she had previously tried non-surgical treatments, but they were unable to address the underlying anatomical changes responsible for her concerns.

After a detailed consultation, we agreed that the most appropriate approach was a deep plane facelift, deep neck reduction and upper blepharoplasty.

By repositioning the deeper facial tissues and addressing the structures beneath the neck, we restored a more defined jawline and neck contour while maintaining a natural, refreshed appearance. The upper eyelids were also rejuvenated to complement the overall result.

These photographs were taken 8 months after surgery. She was thrilled with the outcome, and most importantly, still looks like herself - just fresher and more rested.

Can you have a neck lift without a facelift? Sometimes the answer is yes.

One of the biggest misconceptions in facial rejuvenation is that every ageing neck needs a facelift.

In reality, some patients can achieve an excellent result with an isolated neck lift. We often think about factors such as excess fullness beneath the chin and minimal jowling, but there’s another anatomical feature that is just as important.

The position of the ear relative to the angle of the jaw plays a significant role.

If the ear sits relatively high, a neck-lift incision behind the ear allows enough leverage to tighten not only the neck but often improve the jawline as well.

If the ear sits lower and closer to the jaw angle, the same incision has much less ability to lift the jawline, even if the neck itself can be improved.

This is why two patients with very similar necks may require completely different operations.

The best procedure isn’t determined by age—it’s determined by anatomy.

This 26-year-old patient wanted a result that looked refined and natural, without changing her identity.

Using piezo ultrasonic rhinoplasty, I reduced the dorsal hump, corrected the septal deviation and refined the tip to create a balanced profile while preserving her facial character.

These photographs were taken 2 years after surgery, demonstrating a stable result that has continued to soften and settle beautifully over time.

The best rhinoplasty is one that enhances facial harmony without drawing attention to itself and improves confidence.

A very common rhinoplasty presentation treated with a bespoke solution - piezo rhinoplasty.

This 26-year-old patient wanted a natural result that would soften her profile without changing her identity.

Using piezo ultrasonic rhinoplasty, a number of structural changes were performed to create a more feminine and soft profile. This involved dorsal bump reduction, correction of the septal deviation and refinement of the tip. The aim was to create a nose that looked balanced from every angle while maintaining facial harmony.

These photographs are 2 years after surgery, demonstrating how well a rhinoplasty continues to settle over time. The result is stable, natural and, most importantly, she was thrilled with the outcome.
