It is a pleasure to join colleagues in Morocco for the 2èmes Journées d’Automne de l’Ophtalmologie Marocaine.

Today, I’m exploring two topics that continue to generate important questions in ophthalmology:

• Controversies in Dry Eye Disease
• Dysbiosis in Ocular Surface Disease: What’s New?

From treatment strategies and unresolved debates in dry eye care to the evolving role of the microbiome in ocular surface health, these are areas where discussion and collaboration are essential.

After more then 30 years in ophthalmology, one thing I’ve learned is that great patient care in never about quantity. It’s about quality.

It’s about taking the time, it’s about listening and reassuring, and it’s about making every patient feel that they are in safe hands.

And when a patient takes the time to share an experience like this, there really is no greater reward.

Apparently the clinic doesn’t close on weekends… it just moves home. 🐶👁️😂

Saturday afternoon, minding his own business, and Spencer suddenly finds himself having a full ophthalmic examination.

The downside of having an eye surgeon in the family: no red eye, funny blink or suspicious-looking tear goes unnoticed. 👀😂

Cataracts don’t always start with blurry vision. 👁️

Sometimes the first signs are much more subtle.

Headlights suddenly feel brighter.
Reading isn’t quite as easy as it used to be.
Colours or contrast seem different.
Or perhaps something about your vision has changed — but you can’t quite put your finger on it.

The interesting part? Some of the earliest changes can happen while you still think you’re seeing perfectly well.

Corneal Neuropathic Pain: Part 6

You can’t recover from the wrong diagnosis.

This is perhaps the biggest reason why some patients continue to suffer for years.

They’ve been told it’s dry eye.
They’ve tried all the possible treatments available but the pain remains the same.

Not because they failed but because they were treating the wrong condition.

Corneal Neuropathic Pain is still one of the most overlooked cause of chronic ocular pain. Until we recognise when the nerves (not just tear film) are driving the symptoms, meaningful improvement can remain frustratingly out of reach.

For patients, the message is simple: DON’t LOSE HOPE IF PREVIOUS TREATMENTS HAVE NOT WORKED!

For clinicians, it’s an important reminder: when the signs and symptoms don’t match, it’s time to think differently.

The message is clear. The most powerful treatment is the CORRECT DIAGNOSIS!

New York marked the inaugural DryEyeSociety meeting and what an extraordinary beginning.

As a founding board member, it was a privilege to contribute through two lectures and a practical diagnostic workshop.

My principal lecture, “The Ocular Surface in Focus: Diagnosing Dry Eye in the Next Decade,” explored why diagnosing dry eye is far more complex than performing one test or identifying one abnormal result.

It is about putting the entire clinical picture together: symptoms, tear-film behaviour, inflammation, corneal sensitivity, biomarkers and the many possible drivers of ocular-surface disease.

My second lecture examined conjunctivochalasis, a common but frequently overlooked contributor to ocular-surface symptoms and inflammation.

With more than 500 attendees, outstanding industry support and colleagues travelling from around the world, the energy at this meeting was remarkable.

But perhaps the most important message was this:

The increasing number of dry-eye and ocular-surface societies is not about competition.

Every region brings a different perspective, a different clinical approach and different experience. The more we meet, exchange ideas and challenge one another, the more complete our understanding becomes—and ultimately, the better we can care for our patients.

And with Rolando Toyos the founder of DESA, there is always another important ingredient: fun.

Rolando has a real talent for creating meetings where people do not simply attend lectures and leave. There is always a wonderful social atmosphere, plenty of laughter and time for colleagues from around the world to connect properly outside the scientific programme.

Because conferences are not only about what we learn in the lecture rooms. They are also about the friendships, conversations and future collaborations that begin around them.

A fantastic first meeting for DESA—and a very enjoyable few days in New York.
