ABLATION FOR ATRIAL FIBRILLATION – FOCUS ON CRYOABLATION

Atrial fibrillation ablation is a medical procedure aimed at curing atrial fibrillation (AF or Afib).

In AF, particularly in the form of AF which is intermittent (or paroxysmal AF), the problem is mainly due to extra electrical activity arising from the pulmonary veins which drain blood full of oxygen back into the left atrial chamber. There are 4 pulmonary veins, the left upper and lower, and the right upper and lower veins, and within any of these veins, there can exist cells which can fire rapidly, triggering AF.

The figure below shows a view of the heart from the back (imagine looking at your heart through your spine). Outlined are the left atrium (LA), and the 4 pulmonary veins.

Order Of Valves:

The term "Order of Valves" typically refers to the sequence in which the valves of an internal combustion engine open and close in relation to the engine's piston movement. In a four-stroke engine, there are intake valves, exhaust valves, and the piston, all working together in a specific order.
The four strokes are:

Intake Stroke: The intake valve opens, allowing the air-fuel mixture to enter the combustion chamber as the piston moves down.

Compression Stroke: Both valves are closed, and the piston moves back up to compress the air-fuel mixture.

Power Stroke: The spark plug ignites the compressed mixture, causing a rapid expansion of gases. This forces the piston down, creating power.

Exhaust Stroke: The exhaust valve opens, and the piston moves up, pushing the burned gases out of the combustion chamber.

Cardiac Tumor:
A cardiac tumor is an abnormal growth or mass of tissue that develops in the heart. Tumors can be either benign (non-cancerous) or malignant (cancerous). Cardiac tumors are relatively rare compared to tumors in other parts of the body.

There are two main types of cardiac tumors:

Primary cardiac tumors: These tumors originate within the heart itself. Primary cardiac tumors are extremely rare, and they can be either benign or malignant. The majority of primary cardiac tumors are benign, with myxoma being one of the most common types. Malignant primary tumors include sarcomas.

Secondary (metastatic) cardiac tumors: These tumors occur when cancer from another part of the body spreads (metastasizes) to the heart. The heart is not a common site for metastasis, but it can happen, particularly in advanced cancer cases.

The symptoms of cardiac tumors can vary and may include chest pain, shortness of breath, palpitations, fatigue, and other symptoms related to heart dysfunction. The diagnosis often involves imaging studies such as echocardiography, MRI, or CT scans.

Treatment depends on the type of tumor, its location, and whether it is benign or malignant. Benign tumors may be surgically removed if they are causing symptoms or affecting heart function. Malignant tumors may require more aggressive treatment, such as surgery, radiation therapy, or chemotherapy. In cases of metastatic tumors, the primary focus is typically on treating the cancer in its original site, and cardiac involvement may be managed accordingly.
