SALTER HARRIS FRACTURES

It's time to learn about Salter-Harris Fractures. These are pediatric fractures that involve the epiphyseal growth plate. The mnemonic SALTER can be used to help remember the 5 most common fractures. FYI, this classification system is named after Robert Salter and William Harris who published this classification in 1963.

Type 1 - S: Straight across. fractures occur through the growth plate. These injuries may present with normal radiographs, and the diagnosis is often made clinically when tenderness is palpated over the growth plate.

type II - A: Above. fractures occur through the growth plate and metaphysis. Type II injuries are the most common physeal fractures.

Type III - L : Lower. fractures occur through the growth plate and epiphysis. Type III injuries are intra-articular, and the diagnosis is made radiographically based on the appearance of an epiphyseal fragment not associated with a metaphyseal fracture.

Type IV- T : Through Everything. fractures are also intra-articular and occur through the growth plate, metaphysis and epiphysis.

Type V - R: Rammed. fractures occur when there is a crush injury of the growth plate. The prognosis is poor because growth arrest and partial physeal closure are common.

*DOES FOREFOOT RUNNING CAUSE PLANTAR FASCIITIS?*

A recent article was published (Chen et al. 2019) suggests forefoot running reduces arch height and thus strains the plantar fascia.

However according to DPM, Doug Richie Jr, there are many Methodological errors in this research:

_

1. The arch height was not measured correctly.🤔
In other studies (Kelly, 2014,2016,2012) arch height was measured. They found the height did not change with forefoot strike vs rear foot strike🏃

2. The study did not take into account the intrinsic foot muscles.💪
Kelly states that these intrinsic muscles provide arch control.

3. The plantar aponeurosis supports the arch height and this was not looked at.🔍

-

To conclude:
More research is needed to assess if strain of the plantar fascia occurs with forefoot running, without using arch height to determine this.
