Could a drug-free intestinal adsorbent offer a new direction for bowel urgency in ulcerative colitis?

Faecal urgency can dominate daily life for people with IBD—even when inflammation appears controlled. Work, travel, meals and relationships may all begin to revolve around access to a toilet.

Award-winning proof-of-concept research from Imperial College London tested whether Enterosgel® could adsorb three inflammatory molecules implicated in ulcerative colitis.

Within 15 minutes, Enterosgel adsorbed:

🔬 98.7% of IL-1β
🔬 99.4% of MMP-9
🔬 95.8% of TNF-α

Enterosgel is a non-systemic oral intestinal adsorbent. It acts physically inside the gut and is not absorbed into the bloodstream.

This was an in-vitro proof-of-concept study. It does not prove that Enterosgel treats ulcerative colitis or prevents an IBD flare, but it provides a strong mechanistic rationale for clinical research.

Enterosgel has been available in the UK for over 12 years. It is indicated for acute diarrhoea and IBS with diarrhoea, available from Boots, and listed in the NHS Drug Tariff. Patients can ask their GP whether prescribing is clinically appropriate.

IBS, eczema, hayfever and migraine: four separate conditions — or could there be a common inflammatory link? 👀

When we looked more closely at data from our 440-patient RELIEVE IBS-D clinical trial, we found something unexpected.

📊 Among participants who also had eczema, hayfever, allergic rhinitis, migraine or food intolerance:

52.5% responded to Enterosgel® for their IBS-D symptoms
compared with
28.1% receiving placebo.

In the overall trial population, the responder rates were 37.4% with Enterosgel versus 24.3% with placebo.

Why might people with allergic or atopic conditions have responded more strongly?

🧬 One possible connection is the mast cell—an immune cell found in the gut, skin and around sensory nerves. When activated, mast cells release histamine and inflammatory mediators, including TNF-α and IL-1β.

🔬 In 2025, award-winning student research from Imperial College London found that Enterosgel adsorbed TNF-α and IL-1β in laboratory testing—adding an important piece to the scientific hypothesis.

This does not prove that Enterosgel treats eczema, hayfever or migraine. The subgroup finding was exploratory, and Enterosgel is indicated for IBS with diarrhoea and acute diarrhoea. But the signal is scientifically interesting—and deserves further research.

But they write to me about it.

I’m Elena Markaryan, a mother of three and the founder of ENTEROMED.
