This course does not happen because of me.

Ten years as course director of Fundamental Facelifting Techniques, and it sold out again this year.

Thank you to Dr Kathy Duerksen and Dr Kristin Tarbet for lecturing with me this morning. Thank you to Dr Stuart Seiff and Dr John Ng, President of ASOPRS, and to Kristin, for teaching this afternoon. And thank you to every demonstrator and preceptor who gave up their day so other surgeons could learn.

Three signs someone has had a facelift: a pulled look, a scar in front of the ear, and a changed earlobe. My aim is that you see none of them. Which do you notice most on other people?

$300,000 for a facelift. That is one of the things thousands of surgeons are talking about this week at the world's biggest oculoplastic meeting in New Orleans.

The three biggest conversations:

1. Fillers under the eyes. The feeling here is almost unanimous: avoid them. Swelling and puffiness can last for years, and they can be very hard to remove.
2. The cost of surgery in the US. Some surgeons charge $200,000 to $300,000 for a facelift. Supply, demand and very long waiting lists.
3. Looksmaxxing. My friend and colleague presented on it. Quick fixes sold on social media, and some serious complications.

My takeaway: around the eyes, less is more. The less we do to this delicate anatomy, the more natural it looks.

Too young for a facelift? Maybe not for this one.

She is 45, a busy professional, and she wanted a little rejuvenation, not a big change. She had already tried filler, radiofrequency and polynucleotides.

Because she had very little loose skin, I did an endoscopic facelift: small incisions hidden in her hair, a lift of the cheeks and temples, and a little of her own fat. Twilight sedation, and home the same day.

The honest part: she is not 100% improved, and she still has some lines. It is natural, it is conservative, and it is exactly what she asked for.

I'm a deep plane facelift surgeon. And my own research didn't flatter my favourite operation.

First, the two lifts in plain English. A SMAS facelift tightens the supportive layer that sits under your skin. A deep plane facelift goes one layer deeper and releases that layer, so the cheek, jawline and neck move up together.

This week I presented my analysis of 17 studies and 3,476 patients to answer the question everyone from Hollywood to Harley Street is asking: is the deep plane facelift really better?

Satisfaction: 91% deep plane, 88% SMAS. No significant difference.
Permanent facial nerve injury: 0.09%. Fewer than 1 in 1,000 patients.
Results lasted around 11 years with both.

The technique has a name. The result has a surgeon. Experience, judgement and artistry matter more than the label, and you will never find artistry in a statistics table.

A facelift is one of the most considered investments you will make in yourself. Choose the surgeon before you choose the technique.

"Can I just have a neck lift?" I hear it every week.

Most people notice the neck first. But the face usually comes down with it, which is why the jawline changes and jowls appear. For most of my patients the most natural result comes from lifting the face and neck together.

This patient came to me about her neck. Here she is five months after a deep plane facelift and neck lift.

She only asked about her neck.

So here are the three questions everyone asks me before neck surgery.

1. Can I just have a neck lift? Sometimes, mostly under 40. For most people the best result comes from lifting the face and neck together.

2. Will I have visible scars? In my patients about 80% are discreet and difficult to see, and 15% are virtually invisible. Under 5% heal more slowly or form a raised scar.

3. What is recovery like? About 80% of my patients recover within 2 to 6 weeks. Genetics and lifestyle both play a part.

The honest part: this is real surgery, and nobody heals at exactly the same speed.
