Partial thickness compression sutures are an effective means of hastening the resolution of corneal hydrops. In this video i demonstrate how to place them and adjust them for best effect. I do not use air in the anterior chamber, as it it is not necessary for the resolution of the corneal hydrops, and avoiding air reduces the risk of pupil block and induction of cataract.

This patient underwent penetrating keratoplasty for keratoconus 30 years ago. She developed severe late ectasia affecting the entire 360-degree circumference around the graft, which remained otherwise clear. Contact lens fitting had become unstable and uncomfortable, with best-corrected vision limited to 0.6 logMAR. The extensive thinning ruled out wedge resection, and regrafting would have required an excessively large graft. The resuturing technique shown in this video improved the graft profile, resulting in better uncorrected visual acuity and, most importantly, comfortable contact lens wear with vision of 0.00 logMAR. https://youtu.be/u6lUGGI5_IY?si=C9RaMkw2sqRew5SV

Mix and match with a multifocal lens and a premium monofocal or an EROF lens allows us to get the best of both worlds! This ex-army patient wanted to have good near vision without glasses, but wanted to preserve a very clear distance vision for shooting with his dominant eye. Thanks Graeme Wellard
