Every so often someone posts how they optimised their healthcare by uploading their entire medical history into a ChatGPT project and now ask any question, and how everyone should do the same.

With one blood test, it works beautifully. But that’s the trap.

LLMs weren’t built to consistently extract everything from files. Thirty documents, and something was dropped, you just can’t see what. You can’t see how files were parsed, can’t correct errors, and when you ask a question, you never know what was omitted from the analysis. The longer you live in that Project, the more confidently it starts misremembering (the confidence is the dangerous part).

Why risk while we built the layer that gives you transparency: you can use HDA’s chat grounded just in your data and our KB, no hallucinations, or if you already use Claude/ChatGPR, use HDA as an MCP. Many of the things that are worth tracking for longevity can’t be tracked automatically. But even among the records that you get from clinics and labs we often focus on the population norm rather than our personal baseline and what’s functional for us.

The most valuable health data for preventive care and longevity is surprisingly “boring” and almost never tracked properly:
– what is normal for you
– how fast you recover
– what happened before symptoms started
– early warning signs before illness
– medication effects
– cognitive changes
– days when you feel “off” but can’t explain why

This is the data that helps detect problems early, understand triggers, and get diagnosed faster.

The problem is that this information is rarely captured and even if we do record it somewhere, it’s scattered across notes, apps, PDFs, patient portals, conversations and your memory.

I’ve met enough clinicians to know most of them are doing their best — often under impossible conditions, with whatever information they happen to have in front of them at that moment.
The problem is that the information is almost never complete.
A doctor treating you in London doesn’t see the scan you had in Madrid. The cardiologist doesn’t see what the GP wrote last year. The A&E team at 3am doesn’t see your allergy. Not because anyone failed — because the data lives in 12 different places and nobody thought to bring it together.

Research confirms what patients already know from experience. Fragmented records are directly linked to medication errors, misdiagnosis, delayed treatment and redundant testing (PMC, 2024). Fragmented care is an independent risk factor for mortality — confirmed in a 4.7 million patient study (BMC Medicine, 2023). Cancer patients receiving fragmented care have 26% higher mortality risk than those with coordinated care (ScienceDirect, 2024). 6 to 17% of all adverse events in hospitals globally are caused by diagnostic errors — and the root cause is almost always incomplete information at the point of care (WHO).

This isn’t a technology problem — It’s a sovereignty problem. The patient is the only person who has ever had access to their full health history — and until now, no secure place to centralise it and make it usable.
