What is Taylor Liberator Subcision?

Some acne scars are not simply changes on the surface of the skin. Following severe inflammatory or cystic acne, fibrous bands can form beneath the skin and tether it to the deeper tissues. These bands pull the skin downwards, creating depressed, rolling or boxcar scars.

Because the problem sits beneath the skin, treatments that work only on the surface cannot release this tethering.

Taylor Liberator Subcision is a specialised surgical technique used to cut and release these fibrous bands, allowing the indented skin to lift and creating a smoother facial contour.

The Taylor Liberator is a flat-tipped instrument with protected notches designed to gather and divide the tethering beneath the skin. It is introduced through a small incision near the ear and passed through the superficial subcutaneous plane under tumescent local anaesthesia.

Depending on the types of scarring present, Taylor Liberator Subcision may form the first stage of a wider acne-scar treatment plan:

• Taylor Liberator Subcision releases broad areas of tethered rolling or boxcar scarring.
• Punch excision removes individual deep ice-pick or narrow boxcar scars.
• Fully ablative laser resurfacing can then remodel the remaining surface scarring.

Correctly identifying each scar type is essential, as no single treatment is suitable for every form of acne scarring.

In this video, Dr David Stark performs a face and neck tightening procedure using the InMode QuantumRF 10 device.

This minimally invasive, non-excisional treatment delivers precise bursts of bipolar radiofrequency energy beneath the skin to contract soft tissue and stimulate collagen remodelling.

Potential benefits include:

• Tightening loose or mildly sagging skin across the face and neck
• Improving definition through the lower face and jawline
• Creating a smoother, more contoured facial appearance

Why can lines around the mouth be so difficult to treat?

Often referred to as perioral lines, lip lines, smoker’s lines or barcode lines, these vertical creases can become increasingly noticeable as the skin ages—and they don’t only affect people who smoke.

The mouth is one of the most active areas of the face. Talking, eating, smiling and pursing the lips repeatedly fold the surrounding skin. Over time, the combination of this constant movement, declining collagen levels and cumulative sun exposure can cause these lines to become deeply etched into the skin.

For pronounced perioral lines, superficial treatments may not reach deeply enough to produce a significant improvement. Dr H may recommend deep, fully ablative laser resurfacing around the mouth to remove damaged surface tissue and stimulate the formation of smoother, healthier-looking skin.

Not all raised scars are the same.

A keloid is an abnormal type of scar that continues to grow beyond the original boundaries of a wound or injury.

Unlike a hypertrophic scar, which remains within the original wound margins, a keloid can become raised, firm and bulbous—and may continue to spread over time.

Keloids can also cause uncomfortable symptoms, including:

▪️ Itching and irritation
▪️ Tenderness or pain
▪️ Redness and inflammation
▪️ Increasing thickness or size

They develop when the body fails to recognise when to stop the healing process. Cells involved in wound repair continue producing excessive collagen, resulting in an overgrown scar.

Keloids are more common:

▪️ In darker skin types
▪️ On the ears, chest, shoulders and upper arms
▪️ Following piercings, burns, acne or surgery
▪️ In areas that heal under tension
▪️ Over joints and areas of repeated movement

Because every keloid behaves differently, treatment often requires a carefully planned combination approach. Depending on the scar’s size, thickness, location and activity, this may include:

✔️ Steroid or 5-FU injections
✔️ Cryotherapy combined with injections
✔️ Vascular laser treatment
✔️ Fractional CO₂ or Erbium laser
✔️ Laser debulking for particularly large or bulky scars

Laser debulking alone is not considered a complete keloid treatment, as recurrence is highly likely without additional therapy to control the ongoing scarring process.

Early specialist assessment can help determine the most appropriate treatment plan and manage symptoms as well as the scar’s appearance.

I completed my last case last Thursday. Interestingly a complex Rhinophyma case that had flown over from Boston. I was happy that in our last afternoon we as always made a significant improvement to a patient's life quality through cutaneous laser surgery. After all that is what we do at DrHConsult.

This bank holday I have been looking at the last five years.

Why can’t fully ablative CO₂ laser be used on the neck and chest?

Patients often ask whether the impressive resurfacing and tightening achieved on the face can also be produced across the neck and décolletage.

The reason fully ablative CO₂ is generally avoided in these areas comes down to how the skin heals.

Facial skin contains a greater number of adnexal structures, including hair follicles and sweat and sebaceous glands. These structures act as important reservoirs from which healthy new skin can regenerate after the entire surface has been removed.

The neck and chest contain fewer of these structures. This means they have less regenerative capacity and a significantly greater risk of delayed healing and scarring following aggressive fully ablative treatment.

With fractional laser, microscopic areas of untreated skin remain between the treated areas and assist healing. Fully ablative resurfacing removes the entire surface, so it relies much more heavily on those underlying regenerative structures.

For neck and chest rejuvenation, Dr H may instead recommend a combined approach:

▪️ Cold Ablation to resurface the skin and improve visible sun damage with less thermal trauma.

▪️ Subdermal radiofrequency tightening, such as FaceTite, NeckTite or Quantum—performed by Dr David Stark—to address skin laxity from beneath the surface.

This allows surface damage and underlying laxity to be approached separately, without subjecting the neck and chest to the same level of thermal injury as fully ablative CO₂.

Treatment must always be tailored to the area—not simply repeated across different parts of the body.

Individual suitability and expected results require specialist assessment.

Can different lasers be combined to achieve the best possible result?

Yes—in carefully selected patients, combining laser technologies can create a more tailored and balanced treatment.

Different areas of the face do not always require the same treatment intensity. For example, someone may have deeper lines around the mouth and eyes but significantly less damage across the cheeks.

In this situation, Dr H may use:

▪️ Fully ablative CO₂ laser around the eyes and mouth, where more intensive resurfacing and tightening are required.

▪️ Cold Ablation laser across the remaining face, where a lighter level of resurfacing may be more appropriate.

Because both treatments are ablative, they can be carefully combined to produce a result that remains within a similar overall range—while still giving more attention to the areas that need it most.

The important consideration is balance. Combining an extremely intensive treatment with one that is too light could produce a visible difference between areas, creating the demarcation discussed in our previous Q&A.

A combined approach can therefore provide:

✔️ More intensive correction where it is needed
✔️ Lighter treatment where severe resurfacing is unnecessary
✔️ A treatment plan tailored to different areas of the face
✔️ A more consistent and natural-looking overall result

The aim is not to use as many lasers as possible—it is to select the right technology and treatment intensity for each area of the face.

Melasma isn’t simply sun damage—and it cannot be treated like ordinary pigmentation.

Melasma is a common condition that causes symmetrical brown or grey-brown patches, often appearing across the forehead, cheeks, around the eyes and above the upper lip.

It is driven by overproduction of melanin and may be influenced by:

☀️ UVA exposure
🧬 Genetic predisposition
🤰 Pregnancy and hormonal changes
💊 Contraceptives or hormone replacement therapy

Melasma can be challenging to manage because the pigment-producing cells remain prone to reactivation. This means treatment can significantly reduce the visible pigmentation, but it cannot permanently remove the underlying tendency for melasma to return.

At Dr H Consult, treatment may involve one of two specialist laser approaches:

PicoSure Pro 755nm
Precisely targets unwanted pigment with minimal heat, making it suitable for patients seeking a shorter treatment and lower downtime. A course of approximately two to three sessions may be recommended.

Sciton HALO
A fractional non-ablative resurfacing treatment that can address melasma alongside sun damage, superficial pigmentation, uneven texture and fine lines. It offers more comprehensive rejuvenation but requires a longer recovery period.

Choosing the correct technology, settings and treatment plan is particularly important for darker skin types, where unsuitable or overly aggressive treatment may trigger post-inflammatory hyperpigmentation or worsen melasma.

A consultation and accurate diagnosis are essential, as not every form of facial pigmentation is melasma—and not every laser or device is appropriate.

Can fully ablative CO₂ laser be performed only around the mouth or eyes?

Technically, individual areas can be treated—but with intensive fully ablative resurfacing, it is important to consider how the treated skin will look beside the untreated skin.

If the lines and damaged skin around the mouth are significantly improved while the surrounding face remains untouched, a visible boundary may develop. This is known as a demarcation line.

The treated area could appear:

▪️ Smoother and more renewed
▪️ Lighter or different in colour
▪️ Noticeably different from the surrounding skin

Dr H compares it to applying foundation to only one section of the face and stopping at a sharp line—the contrast can become very obvious.

The same consideration applies around the eyes. Treating one isolated area too intensively without appropriately blending it into the surrounding skin can create an unnatural “new skin versus old skin” effect.

In carefully selected cases, targeted treatment may still be appropriate, but the treatment depth, surrounding skin and blending strategy must all be considered. The aim is not simply to improve one feature—it is to achieve a balanced, natural-looking result across the face.
