If someone you care about is clearly struggling but will not ask for help, you are probably not sure what to do. You may have tried raising it and found that they either minimised it or moved the conversation along.

There are a few things that tend to work better than direct encouragement.

Asking a specific question rather than a general one often gets a different response. Not "are you okay?" but "you seem quieter lately, is there something on your mind?" gives the person something to respond to rather than dismiss.

Sharing relevant information without attaching a directive can also open a door. Letting someone know what a first conversation with a specialist looks like, without adding "you should call them", removes the pressure and leaves them with a choice.

And sometimes the most useful thing is simply making clear that you have noticed and that you are not going to pretend otherwise. People often feel more able to ask for help when they know someone has already seen what they are carrying.

There are some questions worth asking before you start therapy, and most people do not think to ask them until afterwards.

The first is what the therapist specialises in. Therapy is not a uniform profession. Someone who works primarily with relationship difficulties or general life transitions will approach an assessment very differently to someone trained in trauma-focused work or complex presentations. The specialism, when it comes to therapy, does matter.

The second is how they decide which approach to use. A thorough clinical assessment should drive the choice of therapeutic model, not habit or training alone. If the answer is that they use the same approach with everyone, that is worth knowing before you start.

The third is how they measure whether things are improving. Standardised clinical outcome measures are a normal part of accountable therapeutic practice. If outcomes are not being tracked, progress remains subjective on both sides.

NHS psychological therapy is genuinely valuable, and for many people it is the only route. What it cannot offer, in most areas, is anything close to prompt access. Waiting lists for psychological therapy currently run to many months, often longer, and mental health rarely stays static during a long wait. A presentation that could have been addressed in eight to ten sessions at the point it first became a problem may take longer to work through twelve months later, because the secondary effects have had time to build.

What we offers is access at the point when it is most clinically useful. Every person who comes to us goes through a thorough clinical assessment, so the approach is matched to what is actually happening. The clinician working with you has been chosen because their specialism and experience fit your specific situation.
