Overview:
Maintaining a computer data and knowledge of People Planner or similar systems and keeping information up to date at all times.
Working closely with the registered Manager to ensure compliance with all office systems and procedures.
Complete risk assessment and personal centred support plans.
Answering incoming telephone enquiries.
Ensuring all client visits are covered and allocating all Care Assistant to the correct shifts.
Process new clients referrals.
Organise and coordinate weekly rotas and care services.
Work with recruitment to ensure sufficient staffing levels.
Carry out on-call coordinating and response duties outside normal duties outside normal office hours as agreed.
Dealing with and queries and supporting clients and family in a clam and professional manner.
Working with professionals and organisations to ensure person centred packages of care are being delivered.
Ensure that rotas are well planned and organised and to take into account for routes and travel times.
Supporting community care staffing in a managerial position.

Role Description
This role is located in Rochdale, Heywood and Middleton for a Community Care Worker with driving responsibilities. As a Community Care Worker you will provide essential support to individuals in their homes, assist with daily living tasks, and ensure the holistic well-being of service users. Responsibilities include assisting with personal care, meal preparation, medication administration, and transportation to appointments or errands as needed. Building strong, trusting relationships with clients and maintaining accurate records of care are key aspects of this role.

Qualifications
• Experience in caregiving, personal care assistance, and supporting individuals' daily living needs.
• Exceptional communication and interpersonal skills to build trust and rapport with clients and their families.
• Valid driver's license and access to a car with the ability to travel within Rochdale, Heywood and Middleton.
• Organisational skills and the ability to manage time effectively.
• Empathy, patience, and a compassionate approach to care.
• Previous experience in health and social care (desired but not essential).
• Relevant certifications, such as NVQ Level 2 or above in Health and Social Care, are an advantage, however full training and a uniform will be provided
• Competitive rates of pay above NMW along with Holiday Pay and Mileage Allowance

Huge well done to all of our dedicated staff 👏👏 💙💙Afternoon Kobi – I wanted to take the opportunity to thank your staff for delivering an excellent service to my dad, We have visits 3 times a day and your staff present in a very caring way and take on any requests we have. I have to comment that Sundas is one of my dads and the family’s favourite. She always presents with a very mature demeanour and is extremely professional – she is often guiding her colleagues. We would like you to pass on our thanks to her too.

Whenever I have rung the office and spoken with Julie- she has always been extremely helpful and responsive. You do have a lovely team.
