Looking after your heart doesn’t have to be overwhelming. Simple habits can go a long way:

🍏 Eat well - A balanced diet rich in fruits, vegetables, whole grains, and healthy fats
🏃‍♀️ Stay active - Regular movement, even a daily walk, supports cardiovascular health
📈 Know your numbers - Monitoring blood pressure, cholesterol, and blood sugar is key
😴 Prioritise wellbeing - Good sleep and stress management also play a role

📞 Why do our Care Navigators ask questions when you book an appointment?

When you contact the surgery, our Care Navigators may ask a few questions about why you need an appointment. We understand that some patients may find this unusual, but please be assured that they are trained to do this safely and confidentially.

The information you provide helps us make sure you receive the most appropriate care, from the right healthcare professional, at the right time. In some cases, this may mean arranging an appointment with a GP, while in others you may be better helped by a nurse, pharmacist, physiotherapist, social prescriber, or another member of our team.

By understanding the reason for your contact, we can:
✅ Direct you to the most suitable service
✅ Prioritise urgent needs appropriately
✅ Help you access care more quickly
✅ Make the best use of available appointments

All information shared with our Care Navigators is treated confidentially, just as it would be with any member of the practice team.

We are delighted to welcome Kirsten to our Proactive Care Team. Kirsten has a background in acute frailty, acute on the day care (ED), long term conditions and surgery. Bringing a wealth of clinical expertise, compassion and experience, Kirsten will play a key role in supporting patients living with frailty, multiple long-term conditions and complex health needs.

Our Proactive Care Service is designed to provide personalised, preventative support, helping patients stay healthier, more independent and better connected to the care they need.

The service aims to:
✅ Keep people healthier and independent for longer
✅ Reduce crisis-driven care and unplanned hospital admissions
✅ Improve patient experience and continuity of care
✅ Provide equitable support for those with complex health needs
✅ Help ease pressures across the wider health system

By identifying needs early and working closely with patients, carers and healthcare professionals, we can provide the right support at the right time.

This approach places patients at the heart of their care, recognising that early intervention and continuity can make a meaningful difference to long-term health and wellbeing.

This change means your results will be ready for discussion, allowing us to provide personalised advice and support to help you stay healthy.
