A patient was ready to go home at 11am. They didn't leave until 9pm.

Nobody dropped the ball. The doctor signed off. The ward was ready. The transport wasn't.

That small gap is where hospital capacity quietly disappears, and it's more common than most people outside the NHS realise.

Here's how it plays out:

At 2pm, a bed stays occupied because there's no transport to get a medically fit patient home or into care.

By 5pm, that bed is still full, so the next patient can't move up from the ward below.

By 8pm, A&E is at a standstill, with ambulances queuing outside because there's simply nowhere to hand patients over to.

One missed transport slot at 2pm turns into a system-wide bottleneck by evening.
