Somewhere after 40, a woman in fertility treatment starts hearing a sentence about her body.

It is said gently. It is said often. And it is not what the evidence says.

The UK regulator counts every embryo transfer in the country: with donated eggs, live birth rates per transfer are similar for all age groups and stay above 25%. With a woman’s own eggs the same measure falls from 31% under 35 to under 5% at 43 and over. What changed is whose egg, not whose body.

A grade describes how an embryo looks under the microscope on one particular day: how far it has expanded, how the cells that will become the baby and the cells that will become the placenta appear. It is a description, not a prediction. Embryos with a B in their grade go on to become healthy babies, and a perfect-looking embryo carries no promise. The grade helps us choose which embryo to transfer first. It does not tell you how your story ends.

The largest review of the question, nine studies all adjusted for the mother’s age, found that a father’s age is a risk factor in its own right: about a quarter higher at 40 to 44, and 43% higher at 45 and over. The number that matters most to us is none of those. It is that this is information, not blame. His effect is smaller than hers, it is an association rather than a proven cause, and none of it is anyone’s fault. It is one more thing worth knowing when you plan.

The real one is shorter, quieter and far less dramatic than the version they imagined.

You are sedated, so you sleep through the procedure. The eggs are collected with a fine needle guided by ultrasound, usually in about 25 minutes. You wake up, rest for a while, and go back to your accommodation or your home the same day.
