Who needs orthodontic treatment?

It’s a great question, and the answer depends on how we define “need.”

In the strictest medical sense, few people need orthodontic treatment to survive. Most can eat, speak, and function well without it. Aside from certain congenital conditions and significant developmental issues, orthodontic treatment is rarely life-saving.

But healthcare is about more than survival—it’s about quality of life.

Many benefits of orthodontic treatment are difficult to measure. Confidence, self-esteem, social comfort, and feeling happy to smile all matter deeply to patients. Research continues to highlight the positive impact a healthy, attractive smile can have on overall wellbeing.

Orthodontic treatment can also support a healthier environment for the teeth, bite, jaw joints, and airway. While no orthodontist can guarantee improvements in TMJ symptoms, airway function, tooth wear, or long-term oral health, a more balanced and stable bite may benefit some patients.

This patient is a great example of how orthodontic treatment can make a meaningful difference beyond strict clinical criteria.

The patient did not meet the HSE criteria for orthodontic treatment, and from a public healthcare perspective, that decision was appropriate.

However, she was concerned about her smile and experiencing ongoing jaw joint discomfort. Examination revealed a narrow upper jaw, a developing crossbite tendency, and a bite that was not fitting together comfortably.

Treatment involved widening the upper jaw with a hybrid two-screw MARPE appliance, followed by comprehensive braces to align the teeth and refine the bite.

Total treatment time was 16 months.

The result was a broader smile, improved dental arch form, and a more comfortable bite. Most importantly, the jaw discomfort resolved and her confidence improved significantly.

Cases like this remind us that treatment need and treatment benefit are not always the same. While healthcare systems must prioritise care based on clinical need, many patients seek orthodontic treatment for reasons beyond those criteria.

Every bone-borne expander we use is carefully planned using CBCT imaging and digital scans to assess the anatomy, bone availability, root positions, and the safest location for skeletal anchorage.

There’s no single “perfect age” to see an orthodontist.

Some children are brought in far too early. Others are seen far too late.

The reality is that timing depends on the child and the problem.

Certain issues like crossbites, narrow upper jaws, oral habits, tongue posture concerns, or sleep-related breathing issues may benefit from earlier intervention. But not every child needs expansion or early treatment.

Sometimes the best approach is simply careful monitoring and waiting until the right stage of growth.

Orthodontics should never be one-size-fits-all.

Early orthodontic assessment can sometimes make a very important difference in a child’s dental development 👦🦷

This lovely young patient presented with:
• a trapped upper front tooth
• lack of space for the developing upper lateral incisor
• a crossbite caused by a narrow upper jaw

Because the problem was identified early, we were able to guide the developing teeth and jaws using a rapid palatal expander (RPE), followed by gentle alignment of the impacted tooth.

Over approximately 12 months, we achieved a healthy and stable outcome that will support future growth and development.

Importantly, not every child needs early orthodontic treatment. However, early assessment allows us to identify the cases where timely intervention can make treatment simpler and more predictable later on.

Continuing on the potential benefits of palatal expansion in children and adults.

Some evidence suggests links between sleep disordered breathing, mouth breathing, ear health, and jaw function. In selected patients, widening a narrow upper jaw may support better nasal breathing, improved Eustachian tube function, and a more balanced bite.
