When people think about heart disease they often think about blocked arteries. However structural heart disease is a different group of conditions that affects the heart valves and other important structures of the heart. These conditions often develop slowly over many years and may not cause obvious symptoms in the beginning. Regular heart checkups and timely evaluation can help detect these problems before they significantly affect heart function. Early diagnosis allows doctors to monitor progression and determine the most appropriate treatment at the right time.

A patient of mine with breathlessness and fatigue, symptoms often seen in mitral regurgitation, initially made me think of aortic stenosis. But not all valve disease points to aortic stenosis, even when the symptoms appear classic.
Many elderly patients live with mitral or tricuspid regurgitation.
TEER (Transcatheter Edge-to-Edge Repair), including MitraClip-based repair can helps reduce leakage without open-heart surgery in selected patients.
TEER is increasingly recognized as a safe and effective option for mitral regurgitation, and tricuspid TEER is also emerging as an important treatment pathway for selected high-risk patients.

I have met patients who believed they'd reached the end of the road.
They came to me after being told their valve disease was severe, but surgery was too risky for them. For many patients above 65, especially with other health conditions, open-heart surgery carries real risk, and that caution from their surgeons was often the right call. But somewhere between that warning and walking out of the clinic, many patients had translated it into believing nothing more could be done. That's the gap I keep seeing, not a wrong diagnosis, just an unfinished conversation.
Surgery being ruled out does not always mean options have run out.
This is where minimally invasive heart valve treatment comes in. TAVI, Transcatheter Aortic Valve Implantation, can replace a diseased aortic valve through a catheter, without opening the chest. TEER, Transcatheter Edge-to-Edge Repair, can repair a leaking mitral valve, and in select patients, the tricuspid valve, the same way. Both are structural heart interventions built for high-risk and elderly patients, but candidacy depends on the individual's valve condition and anatomy, and is decided only after evaluation by a heart team.

Some experiences just can't be put into words. My Structural Heart Fellowship at the Gottsegen National Cardiovascular Institute (GOKVI), Hungary, is one of them.
It's a strange kind of sad, leaving my patients here for a while. But this is necessary. Not for me. For them.
Advanced structural heart procedures like TAVI are still not widely accessible across our region, so patients often end up traveling long distances right when their bodies need rest and close observation, not a journey home. That gap is what's pushing me to train harder here. It's not a reflection on the incredible work already being done back home.
GOKVI runs one of Europe's most experienced structural heart programs. The mentors here are people I deeply admire, and the curriculum is as intensive as it is exciting. I'm here to absorb everything I can, and I promise to bring it all back, tenfold.

The next wave of structural heart intervention is already taking shape.

TAVI is already expanding beyond just high-risk patients. In the next few years, it will become the standard for most patients with aortic stenosis, including younger, lower-risk patients.
Researchers are also working on:
Valves that last longer (15–20 years instead of 10)
Simpler procedures (even smaller catheters, faster deployment)
Combining TAVI with other treatments (like TEER for multi-valve disease)
Treating patients in their 90s safely
This is already happening in Europe. I'm training at GOKVI, Budapest, to bring this next wave of care to Odisha. So our patients don't have to wait years for it to reach us.

Medicine evolves every year but the responsibility towards patients remains constant. This Structural Heart Fellowship at GOKVI Budapest is a step towards understanding advanced structural heart interventions that are reshaping cardiac care globally. 1 in 4 people over 60 have heart valve disease, and most don't know it. Nearly 1 in 3 patients with severe valve disease can't undergo open-heart surgery due to age or comorbidities. In India alone, 300,000 patients are eligible for TAVI.
TAVI (Transcatheter Aortic Valve Implantation) is a minimally invasive valve replacement that doesn't require open-heart surgery. But the problem is we only have around 30 centres in India, and roughly 7 high-volume centres handle the majority of procedures due to lack of structured nationwide training programs and specialised infrastructure, including hybrid OR and ECMO capability.
I expect to change this for my patients with this upcoming three-month Structural Heart Fellowship at GOKVI Budapest. When I return I can perform TAVI at my centre, help establish a dedicated structural heart team, and expand access to this life-saving minimally invasive procedure for the thousands of patients in India who currently cannot access it.
