PLOT TWIST! 💥 Membangkitkan semula fistula yang mati: Pesakit telah dirujuk untuk ditukar kepada kateter cuff terowong kanan tetapi sebaliknya membetulkan fistula trombosis kiri berusia 3 tahun. Pesakit diberitahu untuk pasrah, tetapi bagi kami, menghidupkan semula adalah hasil yang lebih penting daripada meletakkan pesakit apa-apa jenis kateter. Sempitan adalah 1.32mm. Ia mengalami trombosis 4-6 minggu lalu dan merawat penyumbatan dilakukan dengan jayanya. Pada masa ini, ia telah digunakan untuk 3 kali tanpa sebarang masalah!

Ironic reviving of a dead fistula: Patient was referred for conversion to right tunnelled cuff catheter but got the 3 year old left thrombosed fistula fixed instead.

Patient was told to give up, but for us, reviving it is more important outcome than to put patient on any kind of catheter.

The inflow stenosis was 1.32mm. It was thrombosed 4-6 weeks and un clogging done successfully. Currently used for 3 times without any issue!

令人意想不到的是，一条死气沉沉的动静脉瘘竟然奇迹般地复活了：患者原本被转诊进行右侧隧道式袖套导管置入术，结果却意外地修复了长达三年的左侧血栓形成动静脉瘘。 其他医生曾劝患者放弃治疗，但对我们来说，能够成功修复这条动静脉瘘，比让患者接受任何类型的导管置入术都更为重要。 入流狭窄处直径为1.32毫米。该动静脉瘘已形成血栓4-6周，经成功疏通后，目前已使用三次，未出现任何问题！

Kedai minuman manis sekarang naik macam cendawan selepas hujan.

Satu row kedai pun kadang-kadang boleh jumpa Mixue, Tealive, Chagee, Zus Coffee, Bask Bear, Gigi Coffee, Starbucks, Chatime, Gong Cha, Coolblog, Richiamo Coffee, FamilyMart dan macam-macam lagi.

Dulu minuman manis itu sesekali. Sekarang ia jadi rutin.

Pagi beli kopi susu.
Tengah hari beli matcha.
Petang beli bubble tea.
Malam order frappe p**a.

Masalahnya bukan pada satu jenama. Masalahnya bila minuman manis sudah jadi “air harian”, bukan lagi “treat sekali-sekala”.

Hari ini kita nampak orang beratur panjang di depan kedai air.

10 hingga 20 tahun lagi, sebahagian daripada pelanggan tetap ini mungkin akan mula memenuhi ruang klinik untuk darah tinggi, kencing manis, kolesterol dan obesiti.

30 hingga 40 tahun lagi, ada yang mungkin mula kenal mesin dialisis.

Malaysia sudah cukup bergelut dengan diabetes dan obesiti. NHMS 2023 melaporkan 15.6% orang dewasa di Malaysia menghidap diabetes, manakala 54.4% berada dalam kategori berlebihan berat badan atau obes. Diabetes juga menjadi penyumbang besar kepada kegagalan buah pinggang tahap akhir yang memerlukan dialisis. 

Minuman manis bukan musuh kalau diambil sekali-sekala.

Tapi kalau setiap hari wajib ada satu cup manis sebagai “self reward”, mungkin kita perlu tanya diri.

Adakah ini reward, atau pelaburan senyap untuk penyakit masa depan?

血栓切除机是一个很好的辅助设备，但有时当缺乏有效的现场沟通或信息传递时，作为介入医师，即使没有机器也能完成治疗。

有人说，关键不在于汽车的质量，而在于驾驶员的技术。

Mesin trombektomi adalah alat tambahan yang baik tetapi kadangkala apabila jangkauan fizikal atau maklumat tidak disampaikan, kami sebagai pakar intervensi akan membolehkannya walaupun tanpa mesin.

Ia bukan mengenai kualiti kereta, ia adalah mengenai kemahiran pemandu bak kata sesetengah orang.

Thrombectomy machine is a good adjunct but sometimes when the physical or informative outreach is not conveyed, we as interventionist will make it possible even without the machine.

It is not about the car quality, it is about driver’s skills some say.
