🎉 IVANA CARES – YAKAP MEMBER CARD

We are pleased to introduce the IVANA CARES – YAKAP MEMBER CARD, an initiative of the Ivana Rural Health Unit to strengthen access to accessible, continuous, and people-centered primary healthcare for qualified YAKAP members.

Through this initiative, eligible members registered with the Ivana Rural Health Unit as their Primary Care Provider (P*P) may avail of healthcare services and benefits included under the YAKAP program.

🩺 FREE LABORATORY & DIAGNOSTIC SERVICES

Qualified YAKAP members may avail of laboratory and diagnostic services included in the YAKAP package, based on their health needs and as clinically indicated by their Primary Care Provider.

These services are intended to support early detection, regular monitoring, prevention, and continuity of care.

💊 FREE SELECTED MEDICINES

Qualified members may also receive selected medicines covered under the YAKAP package, subject to program guidelines, eligibility, clinical indication, and availability of medicines.

👤 WHO CAN GET THE IVANA CARES – YAKAP MEMBER CARD?

You may avail of the IVANA CARES – YAKAP Member Card if:

✔️ You are a PhilHealth member, and
✔️ Your registered Primary Care Provider (P*P) is the Ivana Rural Health Unit.

Simply visit the Ivana Rural Health Unit and complete the short registration process to receive your member card.

📋 NOT YET REGISTERED WITH IVANA RHU?

No worries! If your current Primary Care Provider is not yet the Ivana Rural Health Unit, you may visit the RHU and our staff will assist you with the necessary P*P registration or transfer process.

Nasa Ivana na ang Ating Roaming Physical Therapist!

Magandang balita! Ang ating Roaming Physical Therapist ay magbibigay ng konsultasyon at physical therapy services sa Ivana Rural Health Unit sa loob ng dalawang linggo simula bukas.

Maaaring magpakonsulta kung mayroon kang:

* Pananakit ng likod, leeg, balikat, tuhod, o iba pang kasukasuan at kalamnan
* Pagpapagaling matapos ma-stroke
* Pagpapagaling matapos operahan
* Hirap sa paggalaw o problema sa balanse
* Pinsalang dulot ng trabaho o sports

Paalala: Kung mayroon kayong dating medical records, X-ray results, o referral mula sa doktor, pakidala po ang mga ito sa inyong konsultasyon.

Samantalahin ang pagkakataong ito upang makatanggap ng dekalidad na physical therapy services. Inaanyayahan ang lahat ng nangangailangan na magpakonsulta habang narito ang ating Roaming Physical Therapist.
