Please be informed of your scheduled medical examination. Kindly bring the following requirements (original and photocopy):

✅ Chest X-ray (latest, within 3 months)
✅ Complete Blood Count (CBC)
✅ Fecalysis
✅ Urinalysis
✅ Drug Test
✅ Pregnancy Test Result *(for female students only)*

🎓 NEW & TRANSFERRED COLLEGE STUDENTS
✅ Chest X-ray (All Courses)
✅ Drug Test (Additional requirement for BS Criminology students only)
✅ CCSJDM Clinic Form with “Fit to Study/Schooling” and Doctor’s Signature
