Lateral leg pain and numbness are often attributed to L5 radiculopathy, chronic ankle sprains, or compartment syndrome. However, when imaging is unremarkable and ankle stability is normal, consider Superficial Peroneal Nerve Entrapment (SPNE)—a frequently overlooked peripheral nerve entrapment.

🦴 What Is Superficial Peroneal Nerve Entrapment?

The Superficial Peroneal (Fibular) Nerve arises from the common peroneal nerve and travels through the lateral compartment of the leg.

It supplies:

Motor: Peroneus longus and peroneus brevis (ankle eversion)

Sensory: Distal anterolateral leg and most of the dorsum of the foot

Entrapment most commonly occurs where the nerve pierces the crural fascia in the distal third of the leg.

🔬 Pathophysiology

Compression develops as the nerve exits the tight fascial tunnel, usually 5–10 cm above the lateral malleolus.

Common causes include:

Recurrent ankle inversion sprains

Direct trauma

Tight boots or footwear

Fascial thickening or fibrous bands

Repetitive athletic activities

⚠️ Clinical Presentation

Patients commonly report:

✅ Burning pain along the lateral leg

✅ Numbness over the dorsum of the foot

✅ Symptoms worsened by prolonged walking, running, or tight footwear

✅ Mild weakness of ankle eversion (occasionally)

Clinical Pearl: Sensation between the first and second toes remains normal, as this area is supplied by the Deep Peroneal Nerve.

🩺 Clinical Examination

Helpful findings include:

✔️ Positive Tinel's sign over the fascial exit point

✔️ Local tenderness 5–10 cm above the lateral malleolus

✔️ Reproduction of paresthesia with percussion

✔️ Possible small muscle herniation at the fascial opening

Ultrasound or nerve conduction studies may assist when the diagnosis is uncertain.

Because symptoms overlap, SPNE is frequently mistaken for:

L5 Lumbar Radiculopathy

Chronic Lateral Ankle Instability

Common Peroneal Neuropathy

Chronic Exertional Compartment Syndrome

💪 Evidence-Based Rehabilitation

Conservative management includes:

✅ Avoiding compressive footwear

✅ Ankle stability and proprioceptive training

✅ Neural mobilization (nerve gliding)

✅ Biomechanical correction to reduce excessive inversion

✅ Manual therapy and soft tissue release around the fascial tunnel

For persistent symptoms, ultrasound-guided nerve hydrodissection or nerve block may provide relief, while surgical decompression (fasciotomy) is reserved for refractory cases.

📌 Clinical Pearl

If your patient has lateral leg pain with numbness over the dorsum of the foot but normal sensation in the first web space, don't stop at the lumbar spine.

Palpate 5–10 cm above the lateral malleolus, perform Tinel's test, and consider Superficial Peroneal Nerve Entrapment.

Early recognition can prevent months of unnecessary treatment for the wrong diagnosis.
