⚠️ When Vision Changes After Tear Trough Filler: A real case study to help educate you!

Every visual complaint after filler should be treated as a potential vascular emergency until proven otherwise.

Recently, we cared for a patient who developed eye pain, headache, blurred vision, binocular diplopia, and mild ptosis within hours of a very conservative tear trough filler treatment.

Our immediate priority was to rule out vascular occlusion and apply empirical interventions.

Urgent ophthalmologic evaluation after our evaluation and interventions revealed:

✅ Normal retina
✅ Normal optic nerve and disc
✅ No evidence of retinal or ophthalmic artery occlusion
✅ Mild (1 mm) ptosis
✅ Isolated inferior oblique paresis with a subtle limitation in upward gaze

With retinal and optic nerve pathology excluded, the presentation became most consistent with a transient, reversible dysfunction of the inferior oblique muscle or its innervation. Potential mechanisms include transient neurapraxia, localized inflammation, or temporary ischemia affecting the muscle or its small blood supply. While the exact mechanism cannot be proven, the patient’s improving course and reassuring ophthalmologic examination support a favorable prognosis.

Clinical pearls:
✔️ Never dismiss new visual symptoms after periocular filler.
✔️ Rule out retinal and optic nerve involvement first.
✔️ A normal retinal exam does not exclude isolated extraocular muscle dysfunction.
✔️ Early collaboration with ophthalmology is invaluable.

Cases like this remind us that preparation matters just as much as technique. Every complication is an opportunity to improve our clinical judgment and better care for our patients.
