Yesterday, I had the privilege of presenting Dementia 101 in collaboration with Dementia Friendly America, with 9 guests joining us for the conversation.

We discussed recognizing early signs of dementia, safety concerns, caregiver burnout, and—most importantly—how we can better communicate with and support individuals living with dementia and the families who care for them.

One of the messages I hope everyone took away:

We may not always be able to change the reality a person with dementia is experiencing, but we can change how we enter their moment—with patience, respect, and compassion.

Thank you to everyone who attended, asked questions, and contributed to the discussion. Every conversation like this helps us build a more dementia-informed and dementia-friendly community.

Is It Still Safe for Mom to Live Alone With Dementia?

Navigating dementia with an aging parent is a journey filled with quiet worries. You might find yourself checking your phone constantly, or feeling a knot in your stomach every time you drive away from your mom's house. She deeply values her independence, but in the back of your mind, a persistent question keeps repeating; Is it still safe for her to live alone?

Dementia gradually progresses and safety risks often quietly emerge before we fully realize the danger. As adult children, watching for these critical signs can help keep your mom safe:

- Wandering & Getting Lost: Familiar neighborhood routes can suddenly feel unfamiliar. Mom might go for a routine walk or drive to the grocery store and become disoriented or lost.
- Medication Mistakes: Managing complex daily prescriptions becomes confusing. She may forget to take essential dosages or accidentally double-dose because she doesn't recall taking them earlier.
- Leaving Appliances On: Memory gaps turn cooking into a hazard. A stove burner left lit or an iron left unplugged in creates serious fire risks.

🏥 A Safe Discharge Takes More Than a Discharge Plan

This weekend, I had the opportunity to attend the American Case Management Association Conference, and one message stood out:
Getting a patient medically ready for discharge is only part of the equation. What happens when they get home matters just as much.

Case managers are balancing patient safety, family concerns, available resources, length of stay, and the need for efficient, cost-effective transitions.
That reinforced why strong community partnerships are so important.

At Warm Embrace Home Care, we help bridge the gap between discharge and recovery at home— by providing stability, proactive family communication, evolving care, and specialized Dementia & Cognitive support, and the daily assistance families may not be prepared to provide.

My biggest takeaway:
The best discharge plan doesn't end at the hospital door. It connects the patient to the right support at home.

I’m looking forward to building stronger relationships with case managers, discharge planners, social workers, and healthcare professionals who share that goal.
