How dangerous was measles before the vaccine was introduced?

Before the measles vaccine, children with normal vitamin A levels had about a 1 in 93,000 risk of death or permanent disability from measles.

Whether that number seems low, high, or surprising to you, it's worth knowing.

After decades in pediatrics, one thing I’ve learned is that thoughtful parents have been asking many of the same questions for a very long time.

Long before social media.
Long before podcasts.
Long before these conversations became part of the national debate.

They weren’t looking for conflict.
They were looking for information, nuance, and individualized care.

Listening to parents wasn’t the end of the conversation. It was the beginning of asking better questions and taking a closer look at the data.

I’m grateful to the parents who kept asking questions over the years, and to my wife, who encouraged me to look beyond what I had been taught and explore the evidence for myself.

That’s one of the reasons I wrote The Vaccine Book years ago and why I recently updated it.

Here’s a link to my updated Vaccine Book. As part of Amazon’s June Kindle Deal, the Kindle edition is available for $2.99 from June 8–14. https://a.co/d/090b1SCU

If Kindle isn’t your preference, a paperback version is available too here: https://drbobsears.com/books/the-vaccine-book/

Many parents are surprised to learn that transmission and vaccine effectiveness are often more nuanced than public messaging suggests.

For some diseases, vaccinated individuals can still become infected and transmit the illness, sometimes with mild or no symptoms. This has been documented in CDC materials and peer reviewed medical literature.

As Physicians for Informed Consent summarizes:
“Children vaccinated with the diphtheria, tetanus, and pertussis (whooping cough) vaccine (DTaP) or the inactivated polio vaccine (IPV) can still be infected with diphtheria-causing bacteria, pertussis bacteria, or poliovirus and spread them to others, even with mild or no symptoms of their own. [10-13] The influenza vaccines (TIV and LAIV) have not been observed to significantly reduce the spread of influenza. [14,15] About half of schoolchildren vaccinated with the measles, mumps, and rubella (MMR) vaccine can still be infected with measles virus and spread it to others, even with mild or no symptoms of their own. [16-19] Several studies of the COVID-19 vaccine suggest that the vaccine has had no measurable impact on preventing transmission. [20-22]”

This does not mean vaccines have no effect. It means transmission risk is more complex than vaccine status alone.

For additional citations and source references on this topic, see Reason #4 in the resource below. It explains why vaccine status alone does not determine whether someone can catch or spread certain illnesses:

For years, many people were told the unvaccinated were the problem.

But the conversation is more nuanced than most people realize.

Vaccination status doesn’t tell the whole story about transmission.

Dr. Bob Sears joined Alex Clark to discuss what the data show about transmission and why asking questions shouldn’t be controversial.

After decades of caring for pregnant moms, newborns, and families, I’ve learned there are certain things that consistently help birth go more smoothly and certain things that tend to disrupt the process. These 10 steps are simple, practical, and centered around supporting birth as a natural physiologic process while still respecting the importance of safety and good medical care when needed. We’ve talked about how vaccine schedules vary…

But let’s go deeper 👇

If there were one clear “best” schedule, wouldn’t every country follow it?

Instead, we see completely different approaches around the world.

That’s why asking questions isn’t wrong. It’s responsible.
