This past weekend I was back at Duke for the 2026 Flap Course, teaching lumbar artery perforator (LAP) and DIEP flap technique alongside a terrific group of reconstructive microsurgeons from around the country.

Courses like this are how this specialty keeps moving forward — walking through anatomy, dissection, and real cases with faculty and residents, then trading notes on what's working in each of our own practices. Getting to teach it is always a reminder of how much I still learn every time I do.

Grateful for the invitation and for the colleagues who make weekends like this worth the trip.

Honored to have Dr. Chao-Ming Wu in the OR — Chief of the Division of Plastic and Reconstructive Surgery at Chung Shan Hospital in Taichung, Taiwan. He spent two weeks with us to study our approach to breast reconstruction, and was able to witness DIEP, PAP, and LAP flap cases, revision surgery, and implant-based reconstruction.

Exchanging techniques with surgeons from around the world is one of the best parts of this work.

Three patients. Three different flaps. Two days.

Over two days we completed the "trifecta" of autologous breast reconstruction — a bilateral DIEP, a bilateral PAP, and a bilateral LAP flap. Every patient's anatomy and goals are different, and offering the full range of flap options means we can tailor the reconstruction to the individual rather than fitting everyone to a single technique.

The screen here is our tissue oximetry monitor, tracking blood flow and oxygenation to each flap in the hours after surgery.

This week marked a quiet but meaningful milestone — my 500th PAP (profunda artery perforator) flap for breast reconstruction. Every one of those 500 represents a patient who trusted us with one of the most important decisions in her care.

Grateful for the team that makes this work possible, day in and day out. Here's to the next 500.

Over the past 12 months, Dr. Yoo performed 260 flaps — including 175 DIEPs, 51 LAPs, 19 PAPs, and a range of complex reconstructions spanning head and neck, lower extremity, and beyond.

He's heading back to New Orleans to join the Center for Restorative Breast Surgery, another premier breast reconstruction program in the country.

Proud of you, Danny. Go do hard things. Go do great things. 🎓

What does "operative efficiency" actually mean for a breast reconstruction patient?

At this week's GAM meeting in Kelowna, I spoke about efficiency in DIEP flap surgery. It's not about rushing — it's about designing every step of the operation so nothing is wasted: coordinated surgical teams working in parallel, standardized steps the whole OR team knows by heart, and the right setup before it's needed.

The payoff for patients: less time under anesthesia and a smoother path to recovery.

If you have questions about DIEP flap breast reconstruction, send me a message — I'm always happy to help you understand your options.

That's a wrap on my talk at the 45th Annual GAM (Group for the Advancement of Microsurgery), held alongside the Canadian Society of Plastic Surgeons 79th Annual Meeting in Kelowna, British Columbia.

My topic: Microsurgical Efficiency — DIEP Flaps as a Case Study. The short version: when the operative team runs efficiently, patients spend less time under anesthesia, complication risk drops, and recovery starts sooner. That's why we work to refine every step of the process.

Grateful for the warm welcome from my Canadian colleagues. 🇨🇦

A privilege to be part of the 45th Annual GAM (Group for the Advancement of Microsurgery) panel on operative efficiency in microsurgery, alongside Dr. Scott Williamson and Dr. Christiaan Schrag.

The best part of meetings like this is learning from multiple surgeons and their perspective. The very interesting thing is that there were clear themes across all of our practices despite being in totally different locations and focusing on different aspects of plastic surgery. Universal themes were focused on team work, small changes can add up, and a focus on excellence is paramount and not speed.
