A Program of All-Inclusive Care for the Elderly (PACE) provider working to empower people aged 55+ Care Guide Partners PACE is a part of Hosparus Health.

As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

Our PACE participants and team members recently celebrated Twin Day, and it was a day full of creativity, laughter, and connection. From matching outfits and coordinated accessories to perfectly paired personalities, everyone enjoyed showing off their twin-inspired looks.

💜 This CNA Week, we're celebrating the Certified Nursing Assistants who help make it possible for older adults to live independently while receiving the care and support they deserve.

Whether they're assisting with daily needs, offering encouragement, or simply brightening someone's day, our CNAs play an important role in the lives of our participants and their families.
