Wisdom teeth can damage the healthy molar beside them long before pain begins.
When an impacted wisdom tooth grows at an angle toward the second molar, it can create a difficult-to-clean contact area where plaque accumulates. This may increase the risk of decay on the neighboring tooth, particularly along its distal surface near the gumline.
The damage can remain unnoticed until a dental examination or X-ray reveals the problem. In advanced cases, the second molar may require extensive restorative treatment, and severe damage can compromise its long-term prognosis.
The important distinction is that not every impacted wisdom tooth needs extraction. The decision depends on clinical findings, radiographic evidence, the risk of disease, and the condition of the adjacent tooth. Healthy, disease-free wisdom teeth can often be monitored rather than removed routinely.
Pain is not a reliable indicator of whether a wisdom tooth is damaging the neighboring molar.
