High hCG. Empty uterus.

Does that automatically mean ectopic pregnancy?

It’s concerning—but in a medically stable patient, it is not a diagnosis by itself.

The hCG “discriminatory zone” estimates when a gestational sac is likely to be visible on transvaginal ultrasound.

Likely is not the same as guaranteed.

A pregnancy of unknown location—or PUL—is a temporary label, not a final diagnosis. The pregnancy may be:

• Too early to see
• Resolving
• Ectopic

That’s why repeating the hCG and ultrasound is often the plan. Active surveillance isn’t “doing nothing.” It allows the pregnancy to provide enough evidence before an irreversible decision is made.

But symptoms change the equation. Severe or worsening one-sided pain, shoulder pain, dizziness, weakness, fainting, or heavy bleeding require urgent evaluation.

Symptoms outrank the spreadsheet. Always.

Not necessarily.

The belief that hCG must double every 48 hours is one of the most persistent myths in early pregnancy. The expected rise depends partly on the starting value, and some IVF pregnancies that resulted in live births had rises much slower than traditional rules suggest.

Let’s talk about the cup.

A semen analysis may be the most awkward fertility test, but it is also one of the simplest and most useful. No surgery. No anesthesia. No catheter. Just a sample, a clock, and a lab that can give us important information about male fertility.

In this episode of Taco Bout Fertility Tuesday, we break down what actually happens at a semen analysis appointment.

How do you prepare?
Can you collect at home?
What if you miss the cup?
What if you can’t collect in the office?
What does the lab actually check?
And what does it mean if the result is abnormal?

We talk about s***m count, motility, morphology, volume, total motile count, abstinence timing, collection instructions, and why testosterone can actually hurt s***m production instead of helping it.

Most importantly, we talk about why one abnormal semen analysis does not automatically mean infertility.

Sometimes it means we repeat the test. Sometimes it means we look for lifestyle, medication, hormone, or heat-related causes. Sometimes it helps us decide whether IUI, IVF, ICSI, or a reproductive urology referral makes the most sense.

The test may feel awkward, but the information is powerful.
