🌸 Do You Need to Lose Weight Before IVF?

Not necessarily!

Weight can influence fertility and pregnancy health, but delaying IVF to lose weight isn't always the best decision.

Age, ovarian reserve, medical history, and individual circumstances all matter.

The goal isn't simply to reach a number on the scale. It's to make informed decisions about your fertility treatment.

🦠 Can a vaginal swab tell us what's happening inside the uterus?

Not exactly.

The vaginal and endometrial microbiomes may be connected—but they're different environments.

A vaginal test may provide useful clues, but it doesn't give us the complete picture of what's happening inside the uterus.

🌸 Twenty posts. One complicated topic. What should IVF patients really take away from the endometrial microbiome research?

The science is exciting—but it's still evolving.

Throughout this series we've learned:

🦠 Finding bacteria doesn't automatically mean infection.
đź’Š A microbiome result doesn't automatically mean antibiotics.
🔬 Chronic endometritis and the endometrial microbiome are not the same thing.
🧬 The vaginal and uterine microbiomes are different environments.
🌱 Probiotics are promising, but we still need better evidence about IVF outcomes.

And perhaps the most important lesson:

More testing doesn't always mean more answers.

Before another test or treatment, ask:

💬 “What will this change?”

🧬 After several failed embryo transfers, should microbiome testing automatically be your next step?

Not necessarily.

The new 2026 ASRM guidance on recurrent implantation failure reminds us that implantation is complicated. Embryo quality, the uterus, chronic endometritis and other clinical factors may all need to be considered.

The endometrial microbiome is an exciting area of research—but finding a different bacterial pattern does not prove that it caused implantation failure.

Before adding another test, ask:

💬 “What will this test change?”

🦠 Can your vaginal microbiome tell us what's happening inside your uterus?

Maybe it can give us clues—but they're not the same environment.

Researchers are finding associations between vaginal microbiome patterns and IVF outcomes. But a vaginal sample shouldn't automatically be considered a direct picture of the endometrial microbiome.

A better question to ask:

“What will this test actually tell us—and will the result change my treatment?”

🦠 Bacteria were found in your uterus. Does that mean you need antibiotics?

Not necessarily.

Finding bacteria is not the same as finding an infection. And an endometrial microbiome result doesn't automatically mean antibiotics are the answer.

Antibiotics may be appropriate for certain conditions, including chronic endometritis, but treatment should have a clear clinical reason.

A good question to ask your fertility specialist:

“What exactly are we treating—and would treating it improve my IVF outcome?”

Could chronic inflammation in the uterus be affecting implantation — without you even knowing it's there?

Chronic endometritis (CE) can be difficult to recognize because some women have few or no obvious symptoms.

For women experiencing repeated implantation failure or recurrent pregnancy loss, research has explored whether chronic endometritis may be one piece of the puzzle.

The important part? Chronic endometritis can be evaluated and, when diagnosed, treated.

🌸 In my newest article, I explain what chronic endometritis is, how it differs from endometriosis, how doctors test for it, and why it may be worth discussing with your fertility specialist.

🧬 **Should you test your endometrial microbiome before IVF?**

Tests such as **EMMA and ALICE** can provide information about the uterine environment—but does every woman undergoing IVF need them?

Not necessarily.

The more important question may be:

**“Is there something in my IVF history that gives us a reason to investigate my uterine environment?”**

For women with repeated unsuccessful embryo transfers, recurrent pregnancy loss, or concerns about chronic endometritis, this may be a conversation worth having with a fertility specialist.

🌸 Learn what these tests can—and can't—tell you in my latest article.
