We’re out in the community today at the Senior Expo and having such a great time connecting with seniors, families, caregivers, and other local organizations!

At Concho Hearts Hospice, we believe hospice is about more than care at the end of life. It’s about relationships, support, education, and making sure families know they have someone they can call when they need help.

If you’re at the expo today, come by and say hello, grab some goodies, and meet part of the Concho Hearts team!

We’re excited to introduce Jamie Albanese, LMSW, our new Social Worker for Concho Hearts Hospice Houston!

Jamie brings a genuine heart for people and a passion for helping patients and families feel heard, supported, and less alone during difficult seasons. Her background includes healthcare, behavioral health, grief support, crisis intervention, care coordination, and advocacy.

She was drawn to hospice because of the opportunity to build meaningful relationships, support families through difficult transitions, and simply be present when it matters most.

When she’s not working, you’ll probably find her at a concert or Astros game, drinking coffee, traveling, or adding another coffee mug to her collection.

North Houston, we’d love for you to get to know Jamie. If you work in a hospital, skilled nursing community, assisted living, senior living, home health, physician office, social services, or any organization that supports patients and families, reach out and introduce yourself.

Invite Jamie to stop by. Connect with her about resources. Share the needs you’re seeing in your community. Let’s build relationships before families are in crisis so we can better support them when the time comes.
