🔥 When tumor follows the portal venous tree…

18F-DOTATATE PET demonstrates an intensely somatostatin receptor–avid pancreatic neuroendocrine tumor with tumor thrombus extending into the portal vein and its intrahepatic branches.

The branching DOTATATE-avid activity within the portal venous system beautifully delineates the extent of portal venous tumor invasion.

A striking example of how molecular imaging can demonstrate not only the primary neuroendocrine tumor, but also its pattern and extent of vascular involvement.

🩸 Can you spot the bleed?

Tc-99m labeled red blood cell scan demonstrating active gastrointestinal bleeding at the splenic flexure.

On the dynamic images, a new focus of radiotracer activity appears within the colon and subsequently increases in intensity and moves along the bowel lumen—the classic scintigraphic pattern of active GI bleeding.

Dynamic imaging is key: appearance → increasing activity → intraluminal movement.

☢️ When Nuclear Medicine solves the mystery!

This liver-spleen scan with SPECT demonstrates radiotracer uptake within multiple ectopic splenic tissue nodules, confirming splenosis.

🩻 Splenosis results from autotransplantation of splenic tissue, most commonly following prior splenic trauma or splenectomy. These nodules can mimic lymphadenopathy, peritoneal implants, or other masses on conventional imaging.

💡 Teaching point: Demonstration of functioning splenic tissue on Nuclear Medicine imaging can establish the diagnosis and potentially avoid unnecessary biopsy or surgery.

This FDG PET/CT demonstrates intense, symmetric FDG uptake in bilateral hilar lymph nodes, a characteristic pattern seen with sarcoidosis.

🔥 FDG uptake is not synonymous with malignancy. Activated inflammatory cells can also be highly metabolically active, resulting in prominent FDG avidity.

Another great reminder that FDG PET images inflammation as well as cancer! ☢️

☢️ When the HIDA scan tells the story!

This case demonstrates common bile duct (C*D) obstruction on hepatobiliary scintigraphy.

🔎 Key finding: Good hepatic uptake and no biliary excretion of the radiotracer, raising concern for distal biliary obstruction.

📚 A great example of how Nuclear Medicine evaluates physiology, not just anatomy!
