✅ Family Supports Waiver (FSW)
✅ Community Integration & Habilitation Waiver (CIH)

Our compassionate and dedicated team is here to help individuals live more independently while receiving the support they need to thrive.

PAC Services May Include:

✔ Assistance with daily living activities
✔ Personal care and hygiene support
✔ Meal preparation assistance
✔ Community engagement and participation
✔ Support with independence goals
✔ Safety monitoring and supervision
