DOT physical coming up?

A little preparation can help your visit go more smoothly. 📒⭐

Before heading in, gather your driver’s license, current medical card if applicable, and a list of your medications. Remember to bring any glasses, contacts, or hearing aids you use for driving.

Depending on your medical history, you may also need relevant medical records, clearance from your treating provider, or a CPAP compliance report.

👉 You must record serious work-related injuries like:

- Fatalities
- Amputations
- Loss of an eye
- Hospitalizations
- Days away or restricted work
- Medical treatment beyond first aid
