Officially published in the International Journal of Orofacial Myology & Myofunctional Therapy (IJOM) 50th Anniversary Special Issue.

We’re honored to share:
“A Historical Perspective on Orofacial Myofunctional Therapy: Bridging Ancient Practices with Contemporary Clinical Science”

This paper explores the historical evolution of OMT and the growing integration of function, breathing, airway health, sleep medicine, craniofacial development, and interdisciplinary care.

The historical overview and supplemental materials included in this publication provide a brief perspective on some of the educational pathways, contributors, and interdisciplinary developments that helped advance the field.

We are deeply thankful for the many additional individuals, educators, clinicians, researchers, and communities whose contributions have also helped shape modern understanding of function, rehabilitation, breathing, and interdisciplinary care.

We also encourage you to watch Dr. Soroush Zaghi’s IJOM 50-Year Anniversary webinar lecture discussing the historical evolution of the field in greater depth.

Sabrina, congratulations on being accepted to your number one choice for PA school at USC!

You have been such an important part of our team and one of the most dependable, hardworking, and genuinely fun people to work alongside. Through every busy clinic day, every challenge, and every moment where patients, providers, therapists, or referring offices needed support, you were always there with compassion, professionalism, and heart.

Your work ethic, positivity, and dedication have left a lasting impact on all of us. The future is incredibly bright for you, and your future patients will be lucky to have someone who cares as deeply and works as hard as you do.

We are beyond proud of you and excited to watch you take this next step at USC. You earned this through years of commitment, sacrifice, and showing up for others time and time again!

What does “tongue overflow” tell us about tongue space and airway health?

This image highlights a screening tool we use within the FAirEST 6+4 to help evaluate tongue space during Lingual Palatal Suction (LPS). When the tongue extends beyond the teeth on both sides, it can be a strong indicator of insufficient tongue space and may suggest the need for further airway evaluation. Functional screening tools like this help clinicians better recognize patterns connected to sleep-disordered breathing, oral function, and overall development.

Mobility is more than appearance — it’s about function and long-term outcomes. By using measurable functional endpoints and individualized therapy milestones, treatment decisions can be guided with greater precision and consistency to support better patient care and recovery.

Whether evaluating tongue mobility, airway concerns, or oral function, objective assessments help determine timing, necessity, and appropriate treatment for each individual patient.

A functional frenuloplasty is more than a simple tissue release.

At The Breathe Institute, our approach combines precise release and sutured reconstruction with orofacial myofunctional therapy before and after treatment to help restore mobility and support optimal function.

We also recognize that every patient is unique, which is why we may recommend additional supportive therapies and bodywork modalities based on individual needs.

Restricted tongue mobility can impact breathing, sleep, feeding, speech, oral function, and overall quality of life. If you suspect a tongue restriction may be present, visit www.thebreatheinstitute.com to learn more and explore available resources.

Big moment for three incredible members of our team. .gauhar

Minerva Bottomley, Vanessa Aguirre, and Iman Gauhar have each contributed in a meaningful way to our clinical and research operations at The Breathe Institute, supporting Dr. Zaghi and the patients and families we serve every day. Their work has been thoughtful, consistent, and impactful across every level of care.
Now they’re stepping into their next chapter:
Minerva and Vanessa pursuing Physician Assistant school, and Iman continuing on to medical school.
Congratulations to our newest TBI Alumni.
This is the kind of growth you hope to see. People who show up, do the work, and then take that foundation forward to serve at an even higher level.
We’re proud of what you’ve built here and where you’re headed next.

Tongue-tie (ankyloglossia) can subtly affect speech by limiting tongue mobility, especially for sounds that require precise elevation and coordination like “t,” “d,” “l,” “r,” and “th.” In some cases, it shows up as slurring, compensation patterns, or persistent articulation challenges that don’t fully resolve with practice alone.

Not every speech quirk is a tongue-tie. But when mobility is restricted, the body adapts, and those adaptations can carry into breathing, posture, and overall function.

As Obi-Wan might say, “Search your feelings.” If speech takes more effort than it should, there may be a disturbance in the Force.
