A pair of nice dye flow patterns delineating the Sacroiliac Joint to treat this patients bilateral SI Joint pain. I love it when the flow looks perfect like this. Placing contrast during a procedure helps doctors know the medication will go where it’s supposed to!

I have never met any patient who WANTED a spinal fusion surgery. I have also never met any pain doctor or even any spine surgeon who WANTED their patient to undergo a spinal fusion surgery. This is where screws and rods are inserted into someone’s spine to stabilize a spinal segment and stop motion from occurring there, AND It can potentially take a year or more to fully fuse. These surgeries are done when there is not another better choice, and sometimes with good success. However, it’s my job to try to keep patients away from something like this. I’m Dr. Paul Lee, Pain Management Specialist—Non-surgical Orthopaedic, Spine, and Rehab Specialist. There are many options to treat pain prior to considering a fusion surgery. Have you tried all reasonable non-surgical treatments ?

I have to admit, with my background training being in the Pain Management field, surgical skills wasn’t my strongest attribute early on. Most of the time my focus is to help patients find non-surgical ways to treat pain and injuries. But certain procedures require me to call on those skills. Because of that, I worked hard on sharpening them, and now it’s a strength. Shout out to the surgical colleagues who gave me tips along the way. Tag someone you know who has worked at something and has gotten better !

A stellate ganglion block is an injection of medication, typically a local anesthetic, into the area around the stellate ganglion (a bundle of nerves that are part of the sympathetic nervous system for the face and arm) at the front of the neck that can help relieve nerve related pain in the head, neck, upper arm and upper chest.

It’s was named stellate ganglion because it’s shaped (poorly 😅) like a ⭐️ !

It also can help increase circulation and blood supply to the arm.

It’s usually a guided injection with x-ray (pic 1) or with ultrasound (pic 2). There are pros and cons to both approaches.

When done right, one can expect the patient to develop a temporary Horner’s syndrome (pic 4) where the eyelid droops, the pupil constricts, and the eye reddens.

It’s even used to help conditions such as PTSD.

Trialing a peripheral nerve stimulator for low back pain: Sometimes placing a wire over specific nerves on the hip crest will help with long-standing and stubborn back pain. The wires are left in place to stimulate the target area for about 5-7 days to determine if a permanent implant of this style is right for the patient. If it helps, then an implant is scheduled. If it doesn’t, then the wires are removed and the entry site is covered with a bandaid. In this case, the potential implant is about the size of a dime. Will see how this patient does Over the next week.

It’s refreshing to get together to socialize and have fun outside of the office setting, and recognize the contributions of our team members. Without their hard work, us docs wouldn’t be able to treat our patients very effectively. It’s definitely a team effort. Thanks for another great year!
