Most families notice it first as a head preference. A tilt, or flattening on one side.

Torticollis often starts in the neck, but that’s rarely the whole picture. During an eval I’m also looking at head shape, trunk mobility, how they reach and use both sides, rolling and other milestones, infant reflexes, and overall movement patterns.

Each piece tells you something about why the preference is there and what treatment actually needs to address.

The neck is where torticollis shows up. It’s rarely where it ends.

Strength and motor planning are two different skills.

A child can be incredibly strong and still struggle with planning, organizing, and carrying out a new movement.

As a pediatric physical therapist, I’m often paying less attention to whether a child completes the task and more attention to how they solve it. Do they choose an efficient strategy? Can they remember the sequence? Can they adapt when something changes?

Those observations help us understand why a child is struggling and guide treatment that carries over into everyday life—not just the obstacle course in the clinic.

A parent sent me this message the other day.

Two years ago, their son was falling while running.

Last week, he ran a 1 km race in about 4 minutes and finished 7th out of more than 100 kids.

Progress like that doesn’t happen overnight. It comes from a child who keeps showing up, parents who support them every step of the way, and a team that believes in what’s possible.

To every pediatric PT, OT, SLP, teacher, coach, and healthcare professional: on the hard days, remember that the work you do today may change someone’s life in ways you’ll never fully see.
