We focus on prevention, chronic disease management, medication refills, sick visits, and relationship-centered care. Here is an updated version that reflects that the practice is now open and actively seeing patients:
# # About the Practice
Irvin Internal Medicine PLLC is an independent internal medicine practice serving adults and seniors in Mayfield, Murray, and the surrounding Western Kentucky
community. The practice is now open and providing comprehensive, evidence-based primary care with a focus on prevention, chronic disease management, medication management, sick visits, wellness care, and long-term health planning. The vision for Irvin Internal Medicine is simple: unhurried visits, clear communication, and care that values relationships just as much as medical expertise. We believe primary care works best when patients feel known, heard, and supported by a physician who understands their medical history, goals, and life circumstances. Our practice is built around a traditional primary care model with a personalized, relationship-centered approach. We focus on thoughtful medical decision-making, prevention, early intervention, and continuity of care. Whether addressing a new concern, managing chronic conditions, coordinating care with specialists, or helping patients navigate ongoing health needs, our goal is to provide care that feels complete, accessible, and grounded in trust. Irvin Internal Medicine currently sees patients in Mayfield, with plans to establish a permanent Murray location when the right space becomes available. We remain committed to serving both the Mayfield and Murray communities. To support communication and access, the practice uses secure patient communication tools and athenahealth as the electronic medical record system. These systems help support continuity of care, secure documentation, patient messaging, and coordination across care settings.
# # What Makes This Practice Different
Irvin Internal Medicine was built around the belief that primary care should feel connected, responsive, and comprehensive. Our goal is to provide full-scope adult primary care so patients do not feel they must rely on urgent care clinics for concerns that can often be addressed by their own physician. When appropriate, care may be supported through in-person visits, secure messaging, telehealth, or coordination with assisted living facilities for established patients. We focus on quality over volume, clear communication, and long-term relationships. Patients can expect thoughtful visits, evidence-based recommendations, and a care plan designed around their individual needs. As the practice continues to grow, availability for new patients may be limited at times in order to preserve the level of access, responsiveness, and personalized care our patients deserve.
# # Why I Built This Practice
I built Irvin Internal Medicine to create a care model that reflects how I believe medicine should be practiced. Over time, I have come to value not only diagnosing and treating disease, but truly knowing my patients—their stories, their goals, and the context of their lives. I wanted to create a practice that supports thoughtful decision-making, open communication, continuity of care, and a more personal patient experience. This practice allows me to design workflows, communication tools, and visit structures that help me care for patients in a way that feels complete and intentional. My goal is for patients to feel heard, supported, and confident that their primary care physician is accessible and involved in their care. Irvin Internal Medicine exists to provide primary care that is coordinated, compassionate, evidence-based, and centered on the individual patient.
